Contractul subsecvent cu valoarea estimata cea mai mica este de 44.881 lei fara TVA. Contractul subsecvent cu valoarea estimata cea mai mare este de 1.912.184 lei fara TVA.

Licitatia deschisa se va realiza offline cu faza de finalizare LE (1 runda de 1 zi pe 05.11.2009) si se va incheia printr-un acord-cadru cu primii 3 operatori economici declarati castigatori, in ordinea crescatoare a preturilor ofertate pe fiecare lot. Acordul-cadru prevede reluarea competitiei ce se va efectua inainte de incheierea fiecarui contract subsecvent, pentru criteriul "PRET", celelalte conditii ramanand nemodificate. Reluarea competitiei se va face printr-o invitatie de participare la reofertare ce va fi transmisa simultan catre toti operatorii economici semnatari ai acordului-cadru, acestia urmand sa trimita noua oferta in plic sigilat la sediul autoritatii contractante.

Preturile la fiecare lot vor contine in mod obligatoriu 2 zecimale.

Garantie de buna executie a contractului in cuantum de 5 % din valoarea acestuia fara TVA, pentru fiecare contract subsecvent.

* Declaratie de eligibilitate privind neincadrarea in situatiile prevazute la art. 180din OUG 34/2006;
* Declaratie privind neincadrarea in situatiile prevazute la art. 181 din OUG 34/2006;
* Certificat de inregistrare fiscala emis de Oficiul Registrului Comertului copie legalizata, din care sa reiasa ca ofertantul are ca obiect de activitate productia sau comercializarea produselor ce fac obiectul procedurii de achizitie publica.
* Certificat constatator emis de Oficiul Registrului Comertului de pe langa Tribunalul Teritorial de care apartine cu maxim 30 de zile calendaristice inainte de data sedintei de deschidere a ofertelor, prezentat in original sau copie certificata pentru conformitate cu originalul prin semnatura si stampila din care sa rezulte: obiectul de activitate al ofertantului (codul CAEN corespunzator); nu sunt inscrise mentiuni referitoare la condamnari pentru fapte penale, punerea sub stare de intedictie, instituirea curatelei, declararea starii de insolventa sau faliment.

* Declaratie pe propria raspundere privind utilajele, instalatiile, echipamentele tehnice de care dispune si se obliga sa le angajeze operatorul economic pentru indeplinirea contractului.
* Declaratie privind lista principalelor livrari in ultimii 3 ani.
* Autorizatii pentru mijloacele de transport din punct de vedere igienico-sanitar, in functie de specificul produselor ofertate.
* Autorizatia sanitar veterinara de functionare.
Ofertantul va face dovada ca producatorul are implementat si mentine un sistem de management al calitatii conform cerintelor standardului ISO 9001 pentru comercializarea produselor ofertate. Ofertantii producatori trebuie sa faca dovada implementarii sistemului HACCP (Hazard Analisys and Critical Control Point) - standard 20001/2005, conform HG 925/2006 privind siguranta alimentelor. Pentru ofertantii care nu sunt si producatori este necesara prezentarea dovezii implementarii sistemului HACCP de catre producatorul ce le furnizeaza produsele respective. Se accepta documente emise de catre un organism acreditat ce atesta implementarea acestui standard, chiar daca nu a fost obtinut inca certificatul HACCP.
