Please note that this is not a request for quotes or proposals, the Government is seeking information for market research purposes only. The Government may or may not issue solicitation documents. The Department of Veterans Affairs, Illiana Health Care System, 1900 East Main Street, Danville, Illinois 61832 is conducting market research for qualified firms capable of providing all vehicles, personnel, management, transporation, materials, supplies, and equipment necessary to perform contract services as outlines below. Applicable NAICS Code is 621910, Ambulance Services with a size standard of $7.0 million. ,Responses to this notice must be submitted in writing (email or fax acceptable) and recieved not later than 2:00 PM local EST on September 29, 2009. No telephone inquiries will be accepted. Description of work to be performed includes: Furnish 24 hours, 365 days a year if Advanced Life Support, Basic Life Support, Wheelchair van and Hired Car Services, related to Ambulance Services for the Department of Veterans, Illiana Health Care System, 1900 East Main Street Danville Illinois including trips made from a VA Facility to another VA facility, including Community based Outpatient Clinics. Contractor is responsible to hold all required State and local licenses and certifications for drivers and emergency medical personnel. Transports may entail transporting over state lines. The purpose of this notice is to gain knowledge of potential qualified sources and their size classifications (service disabled veteran owned small business, veteran owned small business, Hubzone, 8(a), small disadvantaged business or large business) relative to NAICS 621910 (size standard $7.0 million per annum). Responses to this notice will be used by the Government to make appropriate acquisition decisions. Reponses to this notice should include company/individual name, a service capability statement, examples of similar ambulance services which you have provided services to, DUNS number, street address, point of contact, and the size of your business pursant to the following questions;(1)Is your business large or small? (2)If small, does your firm qualify as a small emerging business? (3)If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a)of the Small Business Act? (4)Is your firm a certified HubZone firm? (5)Is your frim a woman owned or operated firm? (6)Is your firm a certified Services-Disabled Veteran Owned or Veteran Owned Small Business? Reponses to this notice must be submitted in writing (email or fax) and received no later than September 29, 2009. No telephone inquires will be accepted. A solicitation is not currently available. If a solicitation is issued it will be announced on Federal Business opportunities website http://wwwfbo.gov at a later date, and all interested parties must respond to that solicitation announcement seperatly from the reponses to this announcement.
