a. Declaratie privind eligibilitatea, completata în conformitate cu formularul A.
b. Declaratie privind neîncadrarea în situatiile prevazute la art. 181 din OUG nr. 34/2006, completata în conformitate cu formularul B.
c. Declaratie privind calitatea de participant la procedura, completata în conformitate cu formularul C.
d. Certificat de atestare fiscala, pentru persoane juridice, eliberat de Agentia Nationala de Administrare Fiscala, care sa ateste ca ofertantul si-a îndeplinit obligatiile exigibile de plata a impozitelor la bugetul de stat, valabil la data deschiderii ofertelor (prezentat în "original" sau în "copie legalizata").
e. Certificat de atestare fiscala privind impozitele si taxele locale, valabil la data deschiderii ofertelor (prezentat în "original" sau în "copie legalizata").

Information and formalities for evaluating if requirements are met:
a. Se va prezenta bilantul contabil la 31.12.2008, cu viza Administratiei Financiare, în copie xerox.
Se va prezenta ultimul bilant contabil, vizat si înregistrat la Administratia Finantelor Publice sau alte documente emise de ofertantii straini si certificate în sistem international de referinta în materie.
b. Declararea pe propria raspundere a cifrei de afaceri în anii 2007 si 2008.
Cerinta minima: realizarea unei cifre medii anuale de afaceri de cel putin 100.000 €.
În cazul în care ofertantul a dobândit personalitate juridica ulterior anului 2007, cifra medie anuala de afaceri se va calcula prin însumarea cifrelor anuale de afaceri corespunzatoare anilor de existenta a societatii si împartirea rezultatului la nr. anilor de functionare.

1. Ofertantul trebuie sa prezinte cel putin 3 (trei) contracte din partea altor beneficiari cu care ofertantul a avut încheiate contracte de furnizare produse - în ultimii 3 ani (2006,2007, 2008).
2. Documente emise de organisme acreditate, care confirma certificarea sistemului calitatii.
Ofertantul va prezenta documente care sa ateste conformitatea cu standardele si normele de calitate impuse de Comunitatea Europeana (cel putin ISO 9001 sau alte documente echivalente), prezentate în copie legalizata.
