Declaratie de eligibilitate. Declaratie privind neîncadrarea în situatiile prevazute de art. 181 din OUG nr. 34/2006. Declaratie privind calitatea de participant la procedura.Certificat de atestare fiscala pentru bugetul de stat si contributiile sociale, eliberat de catre unitatile teritoriale ale MFP.
Certificat impozite si taxe locale emis la o data ulterioara ultimei scadente de plata, în termen de valabilitate, în original/copie legalizata;Certificatul de atestare fiscala si certificatul de plata impozite si taxe locale se vor prezenta în termen de valabilitate, in original/copie legalizata;
Certificat constatator, în copie, emis în luna în curs sau anterioara datei sedintei de deschidere a ofertelor, de oficiul registrului comertului de pe lânga tribunalul teritorial în raza caruia ofertantul îsi are sediul social, din care sa reiasa:
a) faptul ca ofertantul are în obiectul de activitate prestarea serviciilor care fac obiectul prezentei licitatii;

Autoritatea contractanta are dreptul de a opta pentru achizitionarea serviciilor care fac obiectul licitatiei de la operatorii economici a caror oferte vor fi declarate câstigatoare în cadrul procedurii de atribuire, într-un interval care nu va depasi 3 ani de la atribuirea contractelor initiale, în conditiile art. 122, lit. j) din OUG nr. 34/2006, cu modificarile si completarile ulterioare.
