1.Aviz de functionare emis de Ministerul Sanatatii Publice in termen de valabilitate, care atesta dreptul de a depozita si distribui medicamente de uz uman
2.Autorizare de la producator pentru livrarea produselor ofertate.
Nota: In cazul in care ofertantul este producator va prezenta o declaratie din care sa reiasa aceasta calitate.
3. Declaratie pe proprie raspundere a distribuitorului prin care confirma ca produsele ofertate detin autorizatie de punere pe piata eliberata de catre MSP in conformitate cu Ordinul nr.893/2006 pentru aprobarea Normelor privind autorizarea pentru activitatea de distributie angro de medicamente si Legea 95/2006 privind reforma in sanatate.
4.Declaratie pe proprie raspundere a ofertantului prin care acesta , in caz de adjudecare a ofertei sale, indeplineste toate conditiile sa asigure intreaga cantitate din produsele licitate si castigate, pe toata perioada de derulare a acordului cadru.
