Declaratia de eligibilitate;Declatatie privind neîncadrarea în situatiile prevazute de art.181 din OUG nr.34/2006;Certificat constatator emis de ORC;Certificat de atestare fiscala emis de ANAF;Certificat de atestare fiscala emis de Primaria Oradea, daca ofertantul nu are deschis rol fiscal la Primaria Oradea acesta va depune o declaratie prin care atesta faptul ca nu are bunuri impozabile si nu desfasoara activitati generatoare de obligatii fata de bugetul local al municipiului Oradea.

Cifra de afaceri pe ultimii 3 ani sa fie mai mare sau egala cu 500.000 lei, valoare rezultata exclusiv din furnizare de produse similare;

Bilanturile contabile pe anii: 2006, 2007 si 2008 - din care sa rezulte faptul ca societatea nu a inregistrat pierderi pe parcursul anilor mentionati.
III.2.3) Technical capacity
Information and formalities for evaluating if requirements are met:
* Ofertantul va face dovada existentei unui service propriu sau contract de asociere cu un service autorizat pe raza municipiului Oradea. Pentru service la distante mai mari de 15 km de Oradea, furnizorul va suporta cheltuielile de deplasare pentru autospeciale si pentru conducatorul auto.
* ofertantul va prezenta autorizatie pentru livrarea produselor data de producator;
* Lista cu personal calificat;
* Ofertantul si producatorul vor detine certificat ISO 9001/2001 (sau echivalent)
* Declaratie pe proprie raspundere prin care conducatorul ofertantului atesta faptul ca la elaborarea ofertei s-a tinut cont de obligatiile referitoare la conditiile de munca si protectia muncii.
* Fisa de informatii generale completata în conformitate cu Formularul B2 din prezenta documentatie;
* Fisa privind experienta similara, însotita de:
- copia contractului de achizitie publica
- proces verbal de receptie finala ;
- recomandarea beneficiarului pentru contractul prezentat, recomandare care trebuie sa precizeze cel putin modul de indeplinire a obligatiilor contractuale pe parcursul derularii contractului respectiv;
