THIS IS A SOURCES SOUGHT ONLY; PLEASE DO NOT RESPOND WITH A PROPOSAL. This sources sought notice is issued in an effort to conduct additional market research for information and planning purposes only. The Government is soliciting information to determine the availability of service disabled veteran owned small businesses (SDVOSB) capable of performing the requirement indicated below. The NAICS code is 624310 (Vocational Rehabilitation Services) with a size standard of 7 million dollars. This means that businesses furnishing the counseling services described below, with average annual revenue over the last 3 years under 7 million dollars are generally considered small businesses. The requirement is for the following vocational rehabilitation services: (1) Complete Initial Assessments/Evaluations; (2) Case Management/Rehabilitation Services; (3) Employment Services; (4) Educational and Vocational Counseling; and (5) Discrete Services (1) Complete Initial Assessments/Evaluations: Initial Assessment has four major service groups: (1) Initial Assessment ; (2) Vocational and Medical Assessment; (3) Transferable Skills Analysis and (4) Vocational Exploration. The Contractor shall conduct at least one face-to-face interview with the Veteran on an individual basis. A group session cannot substitute for this individualized session. (2) Case Management: The contractor will provide case management services as required to achieve the objectives of the written rehabilitation plan provided by the VA, to include supportive services, such as addressing medical, financial, academic, and community needs, and preparing Veterans to become job ready. Case Management requires frequent and ongoing services that are provided during Veteran participation on rehabilitation plans to continually address and provide supportive services and track progress in the following areas: academic, job readiness, medical, financial, independent living, and community issues. (3) Employment Services: Employment Services are for those Veterans deemed capable of re-entering the workforce. Services provided are to assist Veterans to obtain and maintain suitable employment consistent with the Veteran's rehabilitation plan. This includes job readiness assistance, rsum development, interview skills and job placement services, and post-placement follow-up. (4) Educational and Vocational Counseling (Chapter 36 and Chapter 35): The Contractor will assist Servicemembers and Veterans to establish employment objectives and outline career counseling/education required to achieve their educational/vocational goals. The following training, rehabilitation, and employment needs shall be addressed by the Contractor for Servicemembers and Veterans receiving Chapter 36 services and to Dependents receiving Chapter 35 services. (5) Discrete Services This Service Group includes independent living (IL) assessments, job coaching, life skills coaching, and job site analysis. For the independent living assessments, a locally licensed occupational therapist (OT) or Certified Rehabilitation Counselor will be required. Licensed Rehabilitation Engineers, Occupational Therapists, or Assistive Technology Specialists are required to provide specialized Rehabilitation Engineering assessments. This announcement is not a request for competitive proposals. All responsible sources shall respond by submitting a written narrative Statement of Capability including detailed technical information and/or other technical informational literature demonstrating the ability to perform the above requirements via email to the point of contacts below. Additionally, the contractor shall address their ability to provide services throughout the entire state of Georgia. No telephone requests will be honored. Contractors must be registered in the Central Contractor Registration (CCR), accessed via the Internet at http://www.ccr.gov. All information provided will become property of the Department of Veterans Affairs and will not be returned. CONTINUATION OF SF 30 Solicitation # VA-316-09-RP-0023 is amended to make the following corrections: 1. Page 9 item C.4 is amended to read: C.4 52.216-19 ORDER LIMITATIONS (OCT 1995) (a) Minimum order. When the Government requires supplies or services covered by this contract in an amount of less than $500, the Government is not obligated to purchase, nor is the Contractor obligated to furnish, those supplies or services under the contract. (b) Maximum order. The Contractor is not obligated to honor-- (1) Any order for a single item in excess of $300,000.00; (2) Any order for a combination of items in excess of $600,000.00; or (3) A series of orders from the same ordering office within 1 days that together call for quantities exceeding the limitation in paragraph (b)(1) or (2) of this section. (c) If this is a requirements contract (i.e., includes the Requirements clause at subsection 52.216-21 of the Federal Acquisition Regulation (FAR)), the Government is not required to order a part of any one requirement from the Contractor if that requirement exceeds the maximum-order limitations in paragraph (b) of this section. (d) Notwithstanding paragraphs (b) and (c) of this section, the Contractor shall honor any order exceeding the maximum order limitations in paragraph (b), unless that order (or orders) is returned to the ordering office within 3 Calendar Days days after issuance, with written notice stating the Contractor's intent not to ship the item (or items) called for and the reasons. Upon receiving this notice, the Government may acquire the supplies or services from another source. (End of Clause) 2. Page 10 item C.5 is amended to read: C.5 52.216-22 INDEFINITE QUANTITY (OCT 1995) (a) This is an indefinite-quantity contract for the supplies or services specified, and effective for the period stated, in the Schedule. The quantities of supplies and services specified in the Schedule are estimates only and are not purchased by this contract. (b) Delivery or performance shall be made only as authorized by orders issued in accordance with the Ordering clause. The Contractor shall furnish to the Government, when and if ordered, the supplies or services specified in the Schedule up to and including the quantity designated in the Schedule as the "maximum." The Government shall order a minimum guarantee of at least $10,000.00 dollars of supplies or services during the base period of this contract. (c) Except for any limitations on quantities in the Order Limitations clause or in the Schedule, there is no limit on the number of orders that may be issued. The Government may issue orders requiring delivery to multiple destinations or performance at multiple locations. (d) Any order issued during the effective period of this contract and not completed within that period shall be completed by the Contractor within the time specified in the order. The contract shall govern the Contractor's and Government's rights and obligations with respect to that order to the same extent as if the order were completed during the contract's effective period; provided, that the Contractor shall not be required to make any deliveries under this contract after 30 April 2012. (End of Clause) 3. Page 40, Section IV - Special Contract Requirements / Guaranteed Minimum (Multiple Award) and Contract Maximum is amended to read: SECTION IV - SPECIAL CONTRACT REQUIREMENTS GUARANTEED MINIMUM (MULTIPLE AWARD) AND CONTRACT MAXIMUM The guaranteed minimum amounts are set forth in the amount of $10,000.00, which applies to the base period only. No minimum quantities are guaranteed for the option periods, if exercised. The Maximum Quantity identified for the base period is $600,000.00. This represents the maximum dollars the Government may expend during EACH contract period (the Base Period, or any of the following option periods, if exercised). 4. Page 67 Attachment C, Schedule of Prices is amended to read: SCHEDULE OF PRICES - estimates based on usages: approx. 50% Atlanta, 10% Augusta 15% Columbus, 10% Tifton, 15% Savannah
