Description: This is not a solicitation announcement. This is a sources sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (service disabled veteran owned small business, veteran owned small business, small business small disadvantage business, woman owned, hubzone, 8(a), or large business) relative to NACIS 624190 (size standard of $7 million)The Louis Stokes Cleveland VA Medical Center is seeking potential sources to provide professional art therapy services eight (8) hours per week, two (2) four (4) hour days between 8:00am-4:30pm; plus supplies to VA Spinal Cord Injury patients located at Wade Park who are either under inpatient or outpatient care. Treatment provided will include all necessary supplies except adaptive equipment. Adaptive equipment will be provided by the SCI OT/PT therapy departments. All art therapy will occur on site at the Wade Park, Louis Stokes, Cleveland Department of Veteran's Affairs Medical Center (LSCVAMC) SCI Center. Art therapy sessions will occur either at the patient's bedside, in their rooms, or in the SCI provided space for group sessions. Art therapist(s) must a master's degree in art therapy. Clinical supervision will be provided through the contractor. Credentialing desired, but not mandatory through the Art therapy Credentialing Board. This contact is effective for a period of one year from the date of award, with one (1) year option. The contract will be subject to the availability of funds. Submittal requirements below: Responses to this sources sought announcement are not a adequate response to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to this sources sought announcement. Responses to this notice should include company name, a service capability statement, three customer references to which you have provided similar services to, copies of applicable licensing and credentialing, product catalog, address, point of contact, and the size of your business pursuant to the following questions: (1) Is your business large or small (2) If small, does your firm qualify as a small, emerging business, or small disadvantaged business? (3) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act? (4) Is your firm a certified hub-zone firm? (5) Is your firm a woman-owned or operated business? (6) Is your firm a certified Service-Disabled Veteran Owned Small or Veteran Owned Small Business? Responses to this notice shall be e-mailed. Other supporting material may be mailed or faxed to the attention of Maladean E. A solicitation is not currently available. Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought announcement, a combined synopsis solicitation announcement will be published in FEDBIZOPPS. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Responses to this sources sought synopsis are not considered adequate responses to any future solicitation announcements. Responses to this sources sought is not a request to be added to a prospective bidders list or to receive a copy of future solicitations. Responses to these notices are not offers and cannot be accepted by the Government to form a binding contract
