Autorizatie (Certificat) de la Miisterul Sanatatii Publice din Romania pentru comercializarea produsele medicale ofertate.
Documentatia tehnica oficiala a producatorului, in limba romana, (prospect produsului, eticheta produsului), din care sa rezulte valorile parametrilor tehnici care fac obiectul evaluarii.
Documentatia tehnica oficiala a producatorului dispozitivelor, in limba romana, din care sa rezulte compatibilitatea liniilor de sange pentru hemodializa si a cartuselor de bicarbonat cu tipul de aparat de dializa, respectiv, a solutiilor cu tipul de mixer (GRANUMIX) pentru care se face oferta.
Autorizare de la producator pentru livrarea produselor;
Nota: In cazul in care ofertantul este producator va prezenta o declaratie din care sa reiasa aceasta calitate.
Dovada certificarii conditiilor de calitate;Ofertantul va prezenta certificatul de acreditare pentru sistemul de management al calitatii ISO 9001 sau echivalent.
NU SUNT ACCEPTATE DECAT PRODUSE CU MARCAJ CE
Produsele care nu au documentatia prezentata complet in forma descrisa vor fi excluse de la licitatie.
Prezentarea mostrelor pentru produsele specificate in caietul de sarcini are caracter obligatoriu
