Declaratie pe propria raspundere privind eligibilitatea din care sa rezulte neîncadrarea ofertantului în situatia prevazuta la art. 180 din O.U.G nr. 34/2006- Formular 12A;
Declaratie pe propria raspundere privind neîncadrarea în prevederile art. 181 din O.U.G nr. 34/2006-Formular 12B;
Declaratie pe propria raspundere din care sa reiasa ca se va tine cont de obligatiile referitoare la conditiile de munca si protectia muncii.
Certificat privind impozitele si taxele locale, în perioada de valabilitate, eliberat de Directia Fiscala
Certificat de atestare fiscala eliberat de Directia Generala a Finantelor Publice, pentru obligatiile de plata la bugetul consolidat în perioada de valabilitate fata de data desfasurarii licitatiei deschise, din care sa rezulte ca ofertantul nu are datorii.
Certificat emis de ORC (functie de locul unde îsi are inregistrat sediul social ofertantul), valabil la data limita de depunere a ofertelor.

Declaratie privind efectivul mediu anual al personalului angajat si al cadrelor de conducere Formularul 12 I.
Se anexeaza CV- uri, diplome, atestate pentru personalul de conducere si personalul de specialitate responsabil cu executia lucrarilor pe fiecare lot in parte, pentru care s-a depus oferta .Personalul de specialitate responsabil cu executia lucrarilor va fi diferit pentru fiecare lot in parte.
Declaratie privind utilajele, instalatiile, echipamentele tehnice. Formular 12 H.
Certificate ISO 9001/2001 sau echivalent, ISO 14001/2001 sau echivalent, OHSAS 18001/2001 sau echivalent.

Ofertantii care se afla în curs de certificare privind implementarea Sistemelor de Management conform standardelor ISO 9001, ISO 14001, OHSAS 18001, nu vor fi calificati în etapa de evaluare a ofertelor. Prezentarea unui document in care se precizeaza ca auditul a fost finalizat , urmand ca operatorul economic sa primeasca certificarea propriu-zisa , poate constitui o dovada privind implementarea Sistemelor de Management conform standardelor ISO 9001, ISO 14001, OHSAS 18001 sau echivalent.
