4. Certificat fiscal privind impozitele si taxele locale în cazul persoanelor juridice eliberat de Primarie in original sau copie legalizata;
5. Declaratie privind calitatea de participant la procedura;
6.Certificat constatator emis de Oficiul Registrului Comertului pentru persoane juridice romane in original sau copie legalizata, sau Autorizatie de functionare pentru persoane fizice romane in original sau copie legalizata;
7.Persoane juridice /fizice straine documente care dovedesc o forma de înregistrare / atestare ori apartenenta din punct de vedere profesional, în conformitate cu prevederile legale din tara în care ofertantul este rezident.

2. Experienta similara;
3. Declaratie privind asigurarea tehnica, din care sa rezulte ca au în dotare sau asigurate de la terti, utilajele si echipamentele necesare realizarii serviciului; Pentru dotarile, utilajele si echipamentele care nu sunt în dotarea proprie, ofertantii vor prezenta contracte sau conventii în original sau copie legalizata cu furnizorii acestora;
4.Declaratie privind asigurarea cu personal, din care sa rezulte ca au angajata forta de munca necesara realizarii serviciilor;
5.Se vor prezenta CV-urile personalului de conducere, precum si a personalului cheie responsabil pentru îndeplinirea contractului;
6.Informatii privind subcontractantii;
7.Informatii privind asocierea;
8.Declaratie de acceptare a conditiilor contractuale. În cazul unei asocieri, aceasta declaratie va fi prezentata în numele asocierii de catre liderul desemnat.

1.Ofertantul va face dovada unei experiente similare constând în cel putin un contract finalizat de proiectare pentru constructii de sanatate. Dovada experientei similare se va face cu urmatoarele documente, în copie: - contractul de servicii; - procesul verbal de predare-primire; - recomandarea din partea beneficiarului. 2.Un arhitect cu experienta în specialitatea studiilor de minim 3 ani.
