a). Declaratie pe propria raspundere privind eligibilitatea din care sa rezulte neincadrarea ofertantului în situatia prevazuta la art. 180 din O.U.G nr. 34/2006- Formular 12A;
b). Declaratie pe propria raspundere privind neincadrarea in prevederile art. 181 din O.U.G nr. 34/2006-Formular 12B.
c). Certificate eliberate de beneficiari sau declaratie pe proprie raspundere ca în ultimii 2 ani si-a îndeplinit obligatiile contractuale si nu a produs prejudicii beneficiarilor.

V.4.1- Declaratie privind calitatea de participant la procedura -Formular 12 C;
V.4.2- Lista principalelor servicii de proiectare, PUZ-uri, prestate in ultimii 3 (trei) ani, continând valori, perioade de prestare, beneficiari (autoritati contractante si clienti privati). Prestarile de servicii se confirma prin prezentarea unor certificate/ procese verbale de receptie emise sau contrasemnate de o autoritate sau de catre clientul privat beneficiar- Formular 12 E;
V.4.3 - Declaratie care contine informatii cu privire la asigurarea cu personalul de specialitate si persoanele responsabile direct de îndeplinirea contractului .
V.4.4. - Ofertantul trebuie sa faca dovada Autorizatiei data de Ordinul Arhitectilor din Romania (L184/2001); dovada înregistrarii in Tabloul National al Arhitectilor din România.
V.4.5 - Certificate care sa ateste faptul ca operatorul/ operatorii economici respecta standardele de asigurare a calitatii se vor prezenta: Certificat SR EN ISO 9001 /2001 sau echivalent
