Declaratii privind eligibilitatea;Declaratie privind neîncadrarea în prevederile art.181;Certificat constatator emis de oficiul registrului comertului sau echivalente.;Certificate necesare pentru producerea si punerea în vânzare (transportarea) a produselor solicitate.;Documentul de înregistrare sanitar-veterinar si pentru siguranta alimentelor si Înregistrarea sanitar veterinara si pentru siguranta alimentelor pentru produse de panificatie;Autorizatii sanitar-veterinare de functionare, valabile în conditiile legislatiei sanitar-veterinare, pentru producerea si transportul laptelui sau a produselor lactate;Autorizatie sanitara de functionare pentru mijloacele de transport utilizate în distributia produselor alimentare;? Licenta de fabricatie pentru fiecare din produsele licitate;certificate constatatoare privind îndeplinirea obligatiilor exigibile de plata a impozitelor si taxelor catre stat, inclusiv cele locale ;

1.cifra de afaceri pe anul 2008 trebuie sa fie minim 50% din valoarea estimata a contractului/contractelor pentru care se înainteaza oferta.

1.declaratie referitoare la efectivele medii anuale ale personalului angajat si al cadrelor de conducere în ultimii 2 ani
2.informatii referitoare la utilajele, instalatiile, echipamentele tehnice de care poate dispune operatorul economic pentru indeplinirea corespunzatoare a contractului de servicii;

1.ofertantul trebuie sa aiba 15 persoane angajate cu atributii referitoare la obiectul contractului (implicati în procesul de productie si transport). 2.ofertantul trebuie sa aiba 5 vehicule sub 3,5 tone sau 3 vehicule peste 3,5 tone care sa poata transporta produsele solicitate. Vehiculele trebuie sa detina autorizatie sanitar veterinara pentru transport alimente 3.dovada detinerii unui contract de prestari servicii cu un laborator acreditat
