1. Aviz de functionare emis de Ministerul Sanatatii Publice in termen de valabilitate, care atesta dreptul de a depozita si distribui medicamente
2. Autorizatie de comercializare de la producator/importator pentru fiecare medicament ofertat.
Nota: In cazul in care ofertantul este producator va prezenta o declaratie din care sa reiasa aceasta calitate.
3. Certificatul de acreditare pentru sistemul de management al calitatii ISO 9001 sau echivalent
4. Declaratie pe proprie raspundere a ofertantului prin care acesta , in caz de adjudecare a ofertei sale, indeplineste toate conditiile sa asigure intreaga cantitate din produsele licitate si castigate, pe toata perioada de derulare a contractului
