Declaratie privind neincadrarea in situatiile prevazute la art. 180 si 181 din OUG 34/2006 Declaratie pe propria raspundere din care sa reiasa ca ofertantii vor respecta regulile obligatorii referitoare la conditiile de munca si de protectie a muncii. Dovada privind indeplinirea obligatiilor: Certificat privind impozitele si taxele locale Certificat de atestare fiscala eliberat de Directia Generala a Finantelor Publice Certificat emis de Oficiul Registrului Comertului sau orice alt act echivalent emis in tara de origine a ofertantului.
Certificate eliberate de beneficiari sau declaratie pe proprie raspundere ca în ultimii 2 ani si-a îndeplinit obligatiile contractuale si nu a produs prejudicii beneficiarilor

Lista principalelor livrari de produse efectuate in ultimii 3 ani, Formular 12 D;
Declaratie pe propria raspundere privind utilajele, instalatiile, echipamentele tehnice, mijloacele de transport pe care ofertantul se angajeaza sa le utilizeze pentru îndeplinirea contractului- Formular 12 H .

Ofertantul trebuie sa faca dovada ca este autorizat sa vânda produsele oferite, sa ofere servicii în perioada de garantie si post-garantie si ca detine competente pentru instalarea, configurarea si mentenanta produselor oferite.

Ofertantul va face dovada ca detine cu orice titlu (acte de proprietate, închiriere, leasing) urmatoarele utilaje:1 buc masina de taiat asfalt; 2 buc masini de interventie ; stand pentru verificarea programelor / modulelor în laborator…1 buc; 1 buc platforma ridicatoare (PRB) Ofertantul va face dovada unui numar de minim 5 (cinci) persoane cu experienta în domeniul instalarii, întretinerii si service-ului. Ofertantul trebuie sa prezinte incheierea a minim un contract de furnizare similar, pentru minim 6 intersectii semaforizate, in valoare de 160.000 lei incheiat în ultimii 3 ani. Declaratie pe propria raspundere din care sa reiasa ca în termen de 15 zile de la data semnarii contractului (în cazul câstigarii licitatiei), furnizorul va detine cu orice titlu pe raza Municipiului Brasov punct de lucru local / Call Center - dispecerat cu telefon fix, fax si telefon mobil pentru managementul si redirectionarea apelurilor de urgenta, privind interventiile de service si mentenanta la respectivele echipamente. Fiecare ofertant va prezenta la data licitatiei: software instalat pe calculator pentru programarea ADC-urilor aflate in proprietatea autoritatii contractante………………………………….1 buc.
