INVITATION TO APPLY FOR ELIGIBILITY AND TO BID
(NEGOTIATED PROCUREMENT)

The Provincial Government of Bukidnon, through its Bids and Awards Committee (BAC), invites suppliers/manufacturers/distributors/contractors to apply for eligibility and to bid for the hereunder projects:

Name of Project : Supply & Delivery of Medical Equipments (P.R. #3)

Description & ABC : For the use of Bukidnon Provincial Hospital of Malaybalay

• Approved Budget for the Contract : Eight Hundred Thousand Pesos (P 800,000.00)
2 units Autoclave Machine, cylinder type horizontal, 200-220kg capacity


• Approved Budget for the Contract : Five Hundred Eighty Eight Thousand Pesos (P 588,000.00)
2 units Defibrillator with monitor


• Approved Budget for the Contract : Three Hundred Fifty Thousand Pesos (P 350,000.00)
2 units Dental Chairs with accessories

Location of the Project : Bukidnon Provincial Hospital of Malaybalay

Project Completion/Duration: : Thirty (30) Calendar Days upon receipt of Notice to Proceed

Prospective bidders should have experience in undertaking a similar project within the last 2 years with an amount of at least 50% of the proposed project for bidding. The Eligibility Check/Screening as well as the Preliminary Examination of Bids shall use non-discretionary "pass/fail" criteria. Post-qualification of the lowest calculated bid shall be conducted.

All particulars relative to Eligibility Statement and Screening, Bid Security, Pre-Bidding Conference Performance Security, Evaluation of Bids, Post-Qualification and Award of Contract shall be governed by the pertinent provisions of R.A. 9184 and its Implementing Rules and Regulation (IRR).
