Garantie de participare: 200 000 lei. Garantie de buna executie: 5% din valoarea contractului de executie.

Orice ofertant este exclus in urmatoarele situatii:
-este in stare de faliment ori lichidare, afacerile ii sunt administrate de un administrator judiciar;
-activitatile sale comerciale sunt suspendate sau fac obiectul unui aranjament cu creditorii;
-face obiectul unei proceduri legale pentru declararea sa in una dintre situatiile de mai sus;
-nu si-a indeplinit obligatiile de plata a impozitelor, taxelor, si contributiilor de asigurari sociale catre bugetul general consolidat;
-a fost condamnat, in ultimii 3 ani, prin hotarare definitiva, pentru o fapta care a adus atingeri eticii profesionale sau pentru comiterea unei greseli in materie profesionala;
-prezinta informatii false sau nu prezinta informatii solicitate de catre autoritatea contractanta;

Se solicita:
- rezultatul exercitiilor financiare pe ultimii 3 ani (extras din bilantul contabil al celor 3 ani)
- declararea cifrei de afaceri pe ultimii 3 ani;
- scrisoare de bonitate bancara din care sa rezulte:
¦ Solvabilitate patrimoniala = 30%;
¦ Lichiditate generala = 100%;
- asigurarea riscului profesional;
- demonstrarea ca operatorul economic are disponibile linii de credit confirmate de banci sau orice alte mijloace financiare suficiente pentru a realiza cashflow de executie a lucrarii

Information and formalities for evaluating if requirements are met:
Se solicita:
- lista principalelor lucrari în ultimii 5 ani
- experienta similara - minim 5 lucrari executate si finalizate, dovedite prin contracte si pv receptie semnate de beneficiar si avizate de IJC;
- recomandari, minim 5;
- informatii referitoare la personalul tehnic de specialitate de care dispune pentru asigurarea controlului calitatii;
- autorizatie ISCIR pentru montaj utilaje de ridicat(ascensoare de persoane);
- informatii referitoare la studiile, pregatirea profesionala si calificarea personalului de conducere precum si a persoanelor responsabile pentru executia lucrarilor;
- declaratie referitoare la efectivul mediu anual al personalului angajat si al personalului angrenat in executia lucrarii;
- atestate RTE;
- dovada dispunerii de laboratoare de incercari materiale;
- dovada certificarii ISO 9001,
- atestatul firmei de executie lucrari de restaurare si conservare monumente istorice - emis de Ministerul Culturii
Informatii privind subcontractantii
Se solicita dupa caz, completarea formularului D cu subcontractantii si specializarea acestora.
Se solicita si pentru subcontractanti:
- atestare de executie lucrari de resataurare si conservare monumente istorice - emis de Ministerul Culturii
