Declaratii privind eligibilitatea:
Cerinta obligatorie: Se completeaza în conformitate cu Formularul 1 de la Sectiunea IV - FORMULARE (Formular nr. 12 A din anexa la Ordinul presedintelui A.N.R.M.A.P. nr. 155/2006).
Declaratie privind neîncadrarea în prevederile art. 181 din ordonanta:
Cerinta obligatorie: Se completeaza în conformitate cu Formularul 2 de la Sectiunea IV - FORMULARE (Formular nr. 12 B din anexa la Ordinul presedintelui A.N.R.M.A.P. nr. 155/2006).
Se solicita confirmarea privind plata taxelor si impozitelor la bugetul de stat si la bugetul si taxele locale, prin prezentarea Certificatului de atestare fiscala privind îndeplinirea obligatiilor exigibile de plata a impozitelor si taxelor catre bugetul de stat si a Certificatului fiscal privind îndeplinirea obligatiilor exigibile de plata a impozitelor si taxelor locale.
Cerinta obligatorie: Certificat constatator emis de Oficiul Registrului Comertului în original sau extras de pe site-ul oficial al Oficiului National al Registrului Comertului (O.N.R.C.) si copie Certificat înregistrarCerinta obligatorie: documente care dovedesc o forma de înregistrare/atestare ori apartenenta din punct de vedere profesionale emis de O.N.R.C.
III.2.2) Economic and financial capacity
Information and formalities for evaluating if requirements are met:
Cerinte obligatorii: Se vor prezenta informatii cu privire la:
1. rezultatul exercitiilor financiare pe ultimii 3 ani. Ofertantul trebuie sa îndeplineasca Cerinta minima de a fi încheiat bilantul contabil cu profit.
2. declararea cifrei de afaceri pe ultimii 3 ani. Ofertantul trebuie sa îndeplineasca Cerinta minima de a avea o cifra de afaceri pe ultimii 3 ani de cel putin 50.000 euro/an, respectiv echivalentul în lei a acestei sume.
Garantie de participare: - cuantumul garantiei de participare: 2.000 lei
- perioada de valabilitate a garantiei pentru participare trebuie sa fie cel putin egala cu perioada de valabilitate a ofertei;
- forma de constituire a garantiei de participare acceptata este prin scrisoare de garantie bancara, completata conform Formularului 5 din Sectiunea IV - FORMULARE (Formular nr. 11 din anexa la Ordinul presedintelui A.N.R.M.A.P. nr. 155/2006) si prezentata cel mai târziu înainte de deschiderea efectiva a ofertelor.

Cerinte obligatorii:
1. Ofertantul trebuie sa îndeplineasca Cerinta minima de a prezenta o lista cu cel putin 3 contracte reprezentând principalele livrari de produse în ultimii 3 ani. Se completeaza Formularul 3 din Sectiunea IV - FORMULARE (Formular nr. 12 D din anexa la Ordinul presedintelui A.N.R.M.A.P. nr. 155/2006).
2. Ofertantul trebuie sa prezinte documente asupra detinerii unui sistem de management al calitatii, certificat de organisme acreditate - în copie.
3. Ofertantul trebuie sa prezinte autorizarea de la producator privind comercialiDaca exista subcontractanti, se solicita completarea Formularului 4 din Sectiunea IV - FORMULARE (Formular nr. 12 G din anexa la Ordinul presedintelui A.N.R.M.A.P. nr. 155/2006) cu subcontractantii si specializarea acestora.zarea si service-ul.
