Garantie de participare in cuantum de 7500 lei. Garantie de buna execcutie in cuantum de 5% din valoarea contractului, fara TVA.

- declaratie de eligibilitate
- declaratie privind neincadrarea in prevedrile art. 181 din oug 34/2006,
- declaratie privind calitatea de participant la procedura,
- certificate constatatoare privind indeplinirea obligatiilor exigibile la plata taxelor, impozitelor si a contributiilor de asigurari sociale
catre bugetul de stat si locale, valabile la data depunerii ofertelor
- certificat de inregistrare CUI si certificat constatator registrul comertului, in termen de valabilitate

-declararea cifrei de afaceri pe ultimii 3 ani

- cifra medie anuala de afaceri în ultimii 3 ani sa fie de pâna la 1.125.000 lei respectiv 308.438,89 EURO (la cursul de 3,6474 lei/euro, din data de 27.02.2008).

- Lista principalelor livrari de produse similare sau identice cu cele ofertate în ultimii 3 ani- formular 12 D
- Ofertantul va prezenta dovada certificarii ISO 9001-9002 pentru sistemul de asigurare a calitatii.
- Ofertantul va prezenta autorizarea de livrare a produselor.
- Ofertantul va prezenta cel putin trei recomandari din partea unui beneficiar/ client din care una sa fie de la o institutie publica.

- îndeplinirea si finalizarea, în ultimii 3 ani a cel putin un contract având ca obiect livrarea de produse similare si care a avut valoarea de 125.000 lei, respectiv 34.270 EURO (la cursul de 3,6474 lei/euro, din data de 27.02.2008)
