NOTICE OF PROPOSED PROCUREMENT (NPP)

Title: Sioux Lookout Zone Dental Services Program for First Nations and Inuit Health (FNIH), Ontario Region of Health Canada - Standing Offer Agreement (SOA)

Introduction

The Ontario Region of Health Canada, First Nations and Inuit Health (FNIH) require dentists, dental assistants, oral and maxillofacial surgeon services, hereinafter referred to as Dental Service Providers in support of the Sioux Lookout Zone Dental Program on an “as and when required” basis, at isolated and occasionally at semi-isolated First Nation communities.

Objective of the Requirement

Health Canada’s (HC) objective is to organize up to seven (7) Standing Offers Agreement(s) (SOAs) with qualified firms to provide Dental Services to communities in Northwestern Ontario on an “as and when requested” basis.

The period for each SOAs will be in effect from the date of award for two (2) years. The Standing Offer(s) may be authorized for use beyond the initial period for two (2) one (1) year optional periods.

Definition of a Standing Offer

A Standing Offer Agreement (SOA) is not a contract. It is an offer from a supplier to provide services on a prearranged pricing basis and under set terms and conditions for a specified period of time on an as-and-when requested basis. A separate Contract is entered into each time a Call-up is made against an SOA. The Crown’s liability shall be limited to the actual value of the Call-ups made within the period specified in the SOA.

Estimated Value

The total value of all Standing Offer Agreements combined resulting from this Request for Standing Offer (RFSO) will not exceed $10,000,000.00 (CAD); including the services acquired, travel if applicable and all applicable taxes.

The above total includes the initial period of the SOAs which will be for two (2) years from date of signing of the Standing Offer Agreement(s), and two (2) one (1) year optional periods which may be exercised at Health Canada’s discretion. The total potential duration of the SOAs may be up to four (4) years.

The overall total of all Call-ups for each year will not exceed $2,500,000.00 including travel if applicable and all applicable taxes.



Background, Assumptions and Specific Scope of the Requirement

Ontario Region, FNIH has a mandate to make certain health care programs and services are available and accessible to First Nations and Inuit in Ontario. In support of this mandate, the Sioux Lookout Zone Office operates a dental program referred to as the “Sioux Lookout Zone Dental Program,” which provides dental treatment and preventive services to registered First Nations people residing in isolated and semi-isolated communities in Northwestern Ontario and to those who can access the main dental clinic facility in Sioux Lookout, Ontario. The Sioux Lookout dental clinic is open Monday through Friday and its hours of service are 8:00 a.m. to 4:00 p.m. Children who require treatment for Early Childhood Caries (ECC) are brought to Sioux Lookout and treated under general anaesthetic at the Sioux Lookout Meno-Ya-Win hospital. Treatment that cannot be provided in either of these venues is referred out to specialists in Thunder Bay, Kenora and Winnipeg. A new clinic with five dental chairs is being constructed in 2009/10 to better accommodate Oral Surgeons for neuroleptic procedures and more simultaneous treatment for routine patients and emergencies from the North. One of the five chairs will be designed as a full-time dental hygiene operatory devoted to prevention services. Smaller dental facilities are located in the Nursing Stations in the North in the communities listed below and dentists and hygienists are flown in on an “as and when requested” basis subject to accommodation in the community. The current method of delivering this service has proven to be the most cost effective means of providing such services to the First Nations communities served by the Sioux Lookout Zone given the only alternative would be to fly the patients out of the community for the services.

Ownership of Intellectual Property

The Intellectual Property arising from any Call-ups will vest with the Crown.

Health Canada has determined that any intellectual property arising from the performance of the work under the Call-ups against the Standing Offer, as per 6.5 of the Treasury Board Policy on Title to Intellectual Property Arising Under a Crown Procurement Contract which states that the Crown may retain ownership of the intellectual property:

"Where the Foreground consists of material subject to copyright with the exception of computer software and all documentation pertaining to that software.” The Offeror(s) shall not disseminate or dispose of said property without permission from Health Canada.

Security Requirements

It is a condition that, prior to proposing any resource for a Call-up against the Standing Offer Agreements (SOAs) resulting from this RFSO, the Contractor and sub-contractors and/or their employees/resources, including replacements, must be security cleared at the “Reliability” status by the Federal Government.

The contractor SHALL NOT remove any government information or assets from the work site(s) and the Contractor shall ensure that its personnel are made aware of and comply with this restriction.






















MANDATORY REQUIREMENTS

Mandatory Requirements are evaluated on a pass or fail basis. Failure on the part of the Bidder to meet any one (1) of the following Mandatory Requirements will result in the proposal being deemed non-compliant and ineligible for any further consideration or evaluation. M1. Firm or Organizational Experience

The Bidder must have a minimum of two (2) years in the last ten years of
corporate experience providing dental services for at least 500 dental
service days per year.

For each relevant experienced, the Contractor must provide the following
information for the experience to be considered for this criteria:

(1) Name and location of the organization in which the service was provided;
(2) Name, address, e-mail and phone number for the client contact point;
(3) Brief description of the services provided;
(4) Beginning and end date of the provision of the services; and
(5) Number of Dental Service Providers supplied.
M2. Performance and Quality Assurance

The Bidder must describe in their proposal the method by which they will measure, track and monitor the performance and quality of the work of their resources, and how they will remedy performance issues.
M3. Dental Service Providers - Dentists

The Bidder must demonstrate their capability to provide Dentists by including in their proposal a minimum of eight (8) resumes that they are prepared to provide as part of their Offer who at the time of the RFSO meet all the minimum qualifications identified in the Statement of Work.

M6. Administrator

The Bidder must identify in their proposal the name of the resource who will act as a liaison between the Contractor and Health Canada and administer the Offer on the part of the Contractor.
M7. Certifications at Annex “A”

Bidders must complete and submit all Certifications at Annex “A” with the bid Health Canada will declare a bid non-responsive if the certifications are not completed and submitted as required.
M8. Aboriginal Businesses Only

The following mandatory criteria applies only to those Bidders who are submitting an offer as an Aboriginal Business:

The Requirements for the Set-Aside Program for Aboriginal Business contained in ANNEX “B”, and the Certification Requirements for the Set-aside Program for Aboriginal Business contained in Annex “C”, shall be submitted with the proposal. It is a precondition to the submission of a valid bid that this certification be accurately completed. Failure to complete and submit the Certification with the proposal will render the proposal ineligible for the evaluation discount
