INVITATION TO APPLY FOR ELIGIBILITY AND TO BID

The Region I Medical Center, through its Bids and Awards Committee (BAC), invites supplier's, distributors and technical experts to apply for eligibility and to bid for the hereunder NEGOTIATED Project with project reference number 08-021:

Name of Project : Equipment packaging and shipment from port of origin (Kaohsiung City, Taiwan R.O.C.) to
port of entry (Manila, Philippines), to port of destination (Region 1 Medical Center, Dagupan
City, Pangasinan) including arrastre and wharfage charges, demurrage fees, trucking fees,
Customs brokerage fees and equipment assembly costs

Location : Region I Medical Center, Dagupan City, Pangasinan

Brief Description : Packaging and shipment of one (1) unit GE single, continuous, spiral HiSpeed CT-SCAN,
including peripheral equipments (Injector, Pb Glass, Manuals etc.) from port of origin
(Kaohsiung City, Taiwan R.O.C.) to port of entry (Manila, Philippines) to port of destination
(Region 1 Medical Center, Dagupan City, Pangasinan), including arrastre and wharfage
charges, demurrage fees, trucking fees, customs brokerage fees and equipment assembly costs.

Approved Budget for
the Contract : Seven Hundred Fifty Thousand Pesos (Php750,000.00)

Date of Delivery : 45 Days
Source of Fund : General Fund

Prospective bidders should have experience in undertaking a similar project within the last two (2) years with an amount of at least 50% of the proposed project for bidding. Only Suppliers duly registered with current and updated records with R1MC shall be allowed to participate in the bidding. Unregistered suppliers must first register at the Suppliers/Contractors Registration Section BAC Office, R1MC, Dagupan City. The Eligibility Check/Screening as well as the Preliminary Examination of Bids shall use non-discretionary "pass/fail" criteria. Post qualification of the lowest calculated bid shall be conducted.

All particulars relative to Eligibility Statement and Screening, Bid Security, Performance Security, Pre-Bidding Conference(s), Evaluation of Bids, Post-Qualification and Award of Contract shall be governed by the pertinent provisions of R.A. 9184 and its Implementing Rules and Regulation (IRR).

Bid documents will be available only to prospective bidders or his authorized representative upon payment of a non-refundable fee of Php3,000.00 to the Region 1 Medical Center Cashier, 2nd Floor, Administrative Building.

R1MC BAC reserves the right to accept or reject any bid without giving any reasons/s, to waive any required formality or technical deficiencies and to terminate the bidding process at any time before contract award, without incurring any liability to the affected parties.
