SECTION VI: COMPLEMENTARY INFORMATION
VI.3) ADDITIONAL INFORMATION: 1. Pre-qualification data required to take
part in this Tender is that set out as a minimum set of data fields needed
for an entry in NHS-sid. The data fields required can be accessed from the
Purchasing and Supply Agency website at www.pasa.nhs.uk. and suppliers
should then register and submit their supplier profile to NHS-sid at
www.pasa.nhs.uk/sid.
2. All potential suppliers should then register their interest with our
Tender Co-ordinator shown above by post, fax or e-mail confirming their
registration details with NHS-sid, and both the EU advertisement reference
and our Tender reference. Should any potential supplier decline
registration on NHS-sid then any registration of interest must be
accompanied by the minimum set of data fields needed for an entry in
NHS-sid. (See above for access).
3. Please ensure that your response contains an e-mail address for use
during this tender exercise.
4. The Trust reserve the option to issue a pre-tender questionnaire
covering further requirements specific to this tender.
5. Where access to NHS-sid is unavailable please email the help desk at

6. Future additions to the successful supplier's product ranges may be
included during the term of the agreement.
7. The Trust reserves the option to extend any contract awarded for up to
twelve (12) months.
8. The Trust undertakes to use its best endeavours to hold confidential
any information provided in any proposal submitted, subject to the Trust’s
obligations under law, including the Freedom of Information Act 1997. If
the applicant considers that any information submitted in any proposal
should not be disclosed because of its sensitivity then this should be
stated with the reason(s) for considering it sensitive.
