1. Ofertantul va completa -Declaratie privind eligibilitatea.
2. Ofertantul va completa -Declaratie privind neîncadrarea în prevederile art. 181 din OUG 34/2006
ofertantul va prezenta Certificatul constatator de atestare fiscala.
Persoanele juridice/fizice straine vor prezenta formulare-tip eliberate de
autoritatile competente din tara în care candidatul/ofertantul este rezident sau
are domiciliul.
3. Ofertantul va completa -Declaratie privind calitatea de participant la procedura.
Mai multe persoane juridice sau fizice au dreptul de a se asocia in scopul
depunerii unei oferte comune.
Certificat constatator emis de oficiul registrului comertului de pe lânga
camera de comert si industrie nationala sau teritoriala valabil pe o
perioada de maxim 30 de zile de la data emiterii.
Pentru persoane juridice/fizice straine:
1. Documente edificatoare care sa dovedeasca o forma de înregistrare
ca persoana juridica sau de înregistrare/atestare ori apartenenta din
punct de vedere profesion

1. Ofertantul va completa -Informatii generale. Cifra de afaceri anuala din ultimii 3 ani: egala sau mai mare decât limita minim impusa de 125818487,40 lei.
2. Asigurare de Raspundere Civila
de pana la 390.000 Euro pe fiecare caz de dauna
3. Ultimul bilant contabil inregistrat de catre o autoritate competenta.

Informatii si formalitati necesare pentru evaluarea respectarii cerintelor mentionate
Ofertantul va completa - Declaratie pe proprie raspundere privind lista principalelor lucrari executate in ultimii cinci ani, serviciile prestate in ultimii trei ani si produsele furnizate in ultimii trei ani.
2. Ofertantul va prezenta cel putin 3 recomandari
3. Ofertantul va completa -Declaratie privind partea/partile din contract care sunt îndeplinite de subcontractanti
4. Declaratie care contine informatii privind dotarile specifice, utilajele,
echipamentul tehnic, mijloace de transport si alte mijloace fixe
5. Obligatii contractuale, in desfasurare fata de alti beneficiari/ clienti.
6. Declaratie pe proprie raspundere privind respectarea conditiilor de munca side protectie a muncii
7. Ofertantul va prezenta documente emise de organisme acreditate, confirma sistem de management al calitatii si de mediu.
8. Ofertantul va completa -Declaratie privind efectivul mediu anual al personalului angajat si al cadrelor de conducere.
