Council, Suffolk Primary Care Trust and Great Yarmouth and Waveney Primary
Care Trust have put their money together in a Pooled Fund to buy services
for people with mental health problems. On behalf of the pooled fund
Suffolk County Council wants to commission a range of community based
services for individuals with mental health problems. The five community
based services being commissioned are Community Recovery Workers, Family
Carers Service, Advocacy, Community Resource Services and Employment
services. All of the services will be expected to promote community
involvement and participation and will promote self care and self
management.

INFORMATION ABOUT LOTS
LOT NO 1
TITLE: Community Recovery Service
1) SHORT DESCRIPTION: Community Recovery Workers will work with adults
suffering from mental health problems to enable them to live as
independently as possible, take control of their lives and access
opportunities that support and maintain recovery. Individuals will be
referred to the service when it has been identified that specific recovery
work is needed to enable them to reach realistic and definable goals that
have been included in their individual support plan and overall care plan.
The service will be delivered through face-to-face contact (visits to the
service user at home or meetings in other locations), telephone support or
appointments at the service providers office premises.

LOT NO 2
TITLE: Employment Service
1) SHORT DESCRIPTION: People with severe mental health conditions have
the lowest employment rate for any of the main groups of disabled people.
Work is an important element in maintaining mental health and promoting
the recovery and wellbeing of those who have experience mental health
conditions. The service will enable people with mental health problems to
maintain and/or re-build meaningful, valued and satisfying lives, even in
the face of ongoing mental health problems. Service providers will work
with all service users to agree individually tailored development plans
and support programmes. The individual support plan should include
measurable goals, plans and timescales specifically related to employment
support. This should be reviewed at a minimum of quarterly intervals. It
will focus on Recovery and introducing and enabling individuals to access
vocational and employment opportunities by building their confidence and
increasing their skills in a planned way. Providers will evidence that the
service user has been involved in setting goals and targets within the
Individual Support Plan. The Individual Support Plan will also include
plans for moving on from the service in a positive manner, that reflects
successful recovery.

TITLE: Family Carers
1) SHORT DESCRIPTION: The aim of the service is to provide a countywide
service to families caring for someone with a mental health need to enable
them to access services in the community, to sustain their caring role,
promote their own health and wellbeing, maintain employment and improve
their quality of life. The service will also enable family carers to
influence the planning and development of services and supports. The
service provider will: — Work with individual family carers to ensure that
they have access to relevant information and are supported in their caring
role — Advocate for family carers of people with mental health needs —
Support Family Carers to work with Pooled Fund Commissioners to plan and
deliver Mental Health Services.

1) SHORT DESCRIPTION: The service will — Promote recovery and enable
people with mental health problems to maintain or re-build meaningful
lives. — Focus on community participation by supporting people to access
existing opportunities in their local communities — Reduce social
isolation by enabling relationships with people outside the mental health
system as well as other users of mental health services — Maximise service
user and carer choice and participation in service operation — Meet the
needs of diverse groups. Services need to be designed so that they can
explicitly meet needs arising from age, gender, ethnicity, religion,
sexuality and disability of black and minority ethnic communities.
Services need to be able to meet the needs of black and minority ethnic
communities including the new and emerging communities in Suffolk. —
Provide access to services for people who have serious mental health
problems including offering ongoing support to those with severe and
enduring problems who need it.

TITLE: Advocacy Service
1) SHORT DESCRIPTION: Advocacy is speaking and taking action with someone
to ensure they can have their wishes heard, make their own choices and
take control over their own lives. Advocacy promotes social inclusion,
equality and justice. The role of the independent advocate will be to both
work with individual service users so that they have a wide range of
information provided in appropriate formats, support them in understanding
the issues causing concern and to explore options and implement the
choices that they makes. Advocates will be directed in their work at all
times by the wishes and interests of the people they advocate for.
