Sealed tenders for the supply of the under mentioned medical equipment will be received by the Chairman, Ministry Procurement Committee, Ministry of Healthcare & Nutrition, 01st Floor, "Suwasiripaya", 385, Rev. Baddegama Wimalawansa Thero Mawatha, Colombo 10, up to 10.00 a.m. on 05.08.2008
Only the sole nominated representative/accredited agent of manufacturers of the equipment offered for the Sri Lanka market are eligible to participate in this bids.
Sealed bids should be made on a complete set of bidding documents in duplicate and original separately. Bid Documents should be obtained from the Senior Assistant Secretary (Procurement), Ministry of Healthcare & Nutrition, 1st Floor, Room No. 150, "Suwasiripaya", 385, Baddegama Wimalawansa Thero Mw., Colombo 10 from 14.07.2008 to 04.08.2008 between 9.30 a.m. and 2.00 p.m. during working days on submission of the following. a) A written application to the Senior Assistant Secretary (Procurement), Ministry of Healthcare & Nutrition, 01st Floor, Room No.150, "Suwasiripaya", 385, Rev. Baddegama Wimalawansa Thero Mawatha, Colombo 10. b) Upon payment of the non-refundable fee stated above in cash to the Chief Accountant, Ministry of Healthcare & Nutrition.
d) Certificate of Business registration e) A certificate / letter issued by the principals appointing your company as local agents for their products f) Upon production of the original a copy of the valid Certificate of Registration issued by the Registrar of Public Contracts "Samagam Medura" D.R. Wijewardhana Mawatha, Colombo 10, should have to be submitted with each offer to be eligible in this procurement.
Sealed bids should be sent by registered post addressed to Chairman, Ministry Procurement Committee, Ministry of Healthcare & Nutrition, 1st Floor, "Suwasiripaya", 385, Rev. Baddegama, Wimalawansa Thero Mw., Colombo 10 or deposited in the tender box kept at the office of the Senior Assistant Secretary (Procurement), at 1st Floor, Room No. 152, and Ministry of Healthcare & Nutrition.
Bids will be opened immediately after the closing of bids at the office of the Senior Assistant Secretary (Procurement), bidders or their authorized agents are permitted to be present when bids are opened.
Bid Documents may be inspected free of charge at the office of the Senior Assistant Secretary (Procurement), given above.
