Declaratii privind eligibilitatea; Declaratie privind calitatea de participant la procedura; Declaratie privind neîncadrarea în prevederile art. 181 din OUG 34/2006+Certificate constatatoare de plata a taxelor si impozitelor la bugetul de stat si cel local, valabile la data deschiderii ofertelor;Certificatul de inregistrare unic emis de camera de comert si industrie; Actul consitutiv (contractul de societate sau statutul);
Certificatul de inregistrare in scopuri de TVA eliberat de MFP - ANAF;
Certificat constatator emis de catre Oficiul Registrului Comertului de pe langa tribunalul teritorial, care sa ateste obiectul de activitate al ofertantului, valabil la data deschiderii ofertelor.

Declaratie pe propria raspundere ca are disponibile resurse reale negrevate de datorii sau linii de credit confirmate de banci sau alte mijloace financiare suficiente pentru sustinerea derularii contractului; Extrase din bilanturile contabile care sa confirme cifrele medii anuale de afaceri declarate precum si inregistrarea lor la administratia financiara.

Cifra de afaceri medie anuala pe ultimii 3 ani trebuie sa fie de cel putin 4.000.000 lei fara TVA.

Information and formalities for evaluating if requirements are met:
Declaratie privind Lista cuprinzind informatii despre contracte similare executate in ultimii 3 ani insotite de certificari de buna executie;Declaratie privind utilajele, instalatiile, echipamentele tehnice de care dispune operatorul economic pentru indeplinirea corespunzatoare a contractului;Pentru Unimog si CAT 428, ofertantul trebuie sa prezinte Autorizatie Tehnica de Functionare emisa de Registrul Auto Român conform O.G. nr. 82 din 24.08.2000, respectiv legea nr. 222/2003 publicata în M.O. partea I, nr. 367/29.05.2003, sau contract de colaborare cu firme autorizate sa execute interventii la acestea; Ofertantul trebuie sa faca dovada ca poseda în dotare proprie sau asigurata prin contracte de închiriere, urmatoarele dotari considerate strict necesare pentru îndeplinirea contractului:
-atelier reparatii dotat cu elevatoare, pod / grinda rulanta
-atelier prelucrari mecanice
-atelier vopsitorie
-banc centricubat pompe injectie
-atelier reparatii electrice;Declaratie privind efectivul mediu anual al personalului angajat si al cadrelor de conducere, conform formularului 8 +CV ale personalului de conducere si ale personalului responsabil pentru indeplinirea contractului de achizitie publica;Autorizatie de livrare din partea producatorului, sau dealer al acestuia, utilajului sau echipamentului din import pentru furnizarea pieselor de schimb.

Ofertantul trebuie sa faca dovada incheierii si indeplinirii in ultimii 3 ani a cel putin unui contract de aceeasi natura si complexitate, cu o valoare de cel putin 1.350.000 lei, insotit de certificari de buna executie.
