Title: Dampness Intervention Study in Schools in New England

NOTICE OF INTENT TO ISSUE A PURCHASE ORDER. The Centers for Disease Control and Prevention, National Institute of Occupational Safety and Health (NIOSH), Division of Respiratory Disease Studies, Laboratory Research Branch, Morgantown, West Virginia hereby announces its intent to issue a purchase order to University of Connecticut for the following services:

NIOSH has ongoing work in New England in regards to indoor environmental quality in schools and possible association with respiratory health. We have a goal of disseminating the use of our respiratory health questionnaire and our semi-quantitative environmental assessment tools in schools in New England. Furthermore we are now in a "Research-to-Practice" phase, in which we will partner with a school district in Connecticut to show what works in remediation of damp schools to improve employee health and environmental measurements. This phase will include environmental and health surveys before and after repair of leaks and damp conditions.
The University of Connecticut's Center for Indoor Environments and Health will
1. Perform communication, outreach, and facilitation functions as required during planning and progress of the dampness intervention study
2. Assist in field surveys in Bridgeport school district facilities with the application of the health and environmental surveillance tools developed by NIOSH.
3. Convene stakeholder meetings at the school district as a means to communicate the progress of NIOSH work in schools and solicit comment.

This project is supported by justification for other than full and open competition to The University of Connecticut under authority of 41 U.S.C.253(c)(1). Responsible sources that believe they possess the expertise and capabilities identified above are encouraged to submit to the Contracting Officer within 15 days from the posting date of this notice, their written capabilities statement and pricing information in the format they choose, not to exceed 10 pages. Please forward the capability statement and pricing information to Rebecca Mullenax, Purchasing Agent, CDC, NIOSH, 1065 Willowdale Road, Morgantown, WV 26505 or e-mail RMULLENAX@CDC.GOV or Fax to 304-285-6083. Please reference # 000HCCHB-2008-59007; all vendors must be registered in the Central Contractor Registry (CCR) prior to an award of a federal contract. The website is: www.ccr.gov.

The Government will review any/all capabilities statements and pricing information submitted and determines if other qualified sources do exist that could provide this requirement. Information received in response to this announcement will be used solely for the purpose of determining whether to conduct a competitive procurement.

If no affirmative responses are received within 15 days, in accordance with FAR 13.106-1(b)(1), negotiations will be conducted with The University of Connecticut, 263 Farmington Ave 3, Farmington, CT 06032-1956 as the only source and a purchase order will be issued without any additional notices being posted.
