Request for Proposal - Physiotherapy Services for Long Term Care Homes

The Community and Protective Services Department of the City of Ottawa, hereinafter referred to as the City, is seeking proposals to provide a comprehensive physiotherapy program including equipment for the residents in Long Term Care Homes.

The City of Ottawa’s Long Term Care (LTC) facilities requires a service provider to deliver a seamless and comprehensive physiotherapy program. The program is for the assessment and treatment of a range of medical conditions to improve the movement and function of the residents in Long Term Care.

These services are required at the City’s four (4) LTC Homes:
-    Peter D. Clark Centre, 9 Meridian Place, Nepean, Ontario K2G 6P8
-    Garry J. Armstrong LTC Home, 200 Island Lodge Road, Ottawa Ontario K1N 5M2
-    Centre d’Accueil Champlain, 275 Perrier Street, Vanier, Ontario K1L 5C6
-    Carleton Lodge, 55 Lodge Road, R.R. #2, Nepean Ontario K2C 3H1

Period of Proposed Contract:
The proposed period of contract shall be for an initial duration of four (4) years and will commence from the date of contract award. It is anticipated that the contract will start on or around 01 September 2008. The City reserves the right to extend the contact for one (1) additional year

Inquiries:
All inquiries regarding this Request for Proposal (RFP) are to be directed to the Contracting Authority specified herein. Inquiries must be received in writing (e-mail) no later than Wednesday 23 July 2008. All inquiries received, and the answers are provided by the Project Authority will be provided to all Proponents by way of written addendum, no later than Monday, 28 July 2008 without naming the source of the inquiry.

The bid document MUST be received NO LATER than 3:00 p.m. local time,
Wednesday, 06 August 2008.
