Se solicita declaratie privind cifra medie de afaceri pe ultimi 3 ani, in domeniul de activitate aferent obiectului contractului.
Minimum level(s) of standards possibly required
Se va completa si depune, in original, Formularul A din Sectiunea Formulare. Cifra medie anuala de afaceri pe ultimi 3 ani rezultand din activitatea corespunzatoare obiectului procedurii trebuie sa fie egala sau mai mare de 1.900.000 lei. Nota In situatia in care ofertantul a fost infiintat sau si-a inceput activitatea economica de mai putin de 3 ani, cifra de afaceri se va calcula corespunzator perioadei efective de activitate.

Se solicita lista a principalelor prestari de servicii similare efectuate in ultimii 3 ani, continand valori, perioade de livrare, beneficiari, indiferent daca acestia din urma sunt autoritati contractante sau clienti privati si prezentarea unui contract de servicii anterior ce reprezinta din punct de vedere valoric suma de 200.000 lei sau prezentarea a trei contracte anterioare a caror suma cumulata sa fie de 320.000 lei. Prestarile de servicii efectuate se confirma prin prezentarea de recomandari din partea autoritatii sau a clientului privat beneficiar.

Se va completa si depune, in original, Formularul B din Sectiunea Formulare. Se va anexa cel putin o recomandare emisa de beneficiari, la care se face referire in formularul C, in original sau copie.
