Declaratie privind eligibilitatea;Declaratie privind calitatea de participant la procedura; Certificat constatator eliberat de Ministerul Justitiei sau echivalent din care sa rezulte nume complet,sediul,persoanele autorizate/administratori;Adresa de la Curtea Districtuala/Instanta Competenta/Agentie Guvernamentale unde îsi are sediul compania/sucursala sau echivalent din care sa rezule ca operatorul economic nu este in insolventa/faliment/aranjament cu creditorii;Certificat de atestare fiscala eliberat de organul de administrare fiscala al unitatii administrativ teritoriale pe raza careia societatea îsi are sediul social privind plata obligatiilor la bugetul de stat sau echivalent,valabil la data deschiderii ofertelor;Certificat privind plata impozitelor si taxelor locale, valabil la data deschiderii ofertelor;Certificat pentru plata CAS(pentru statele în care autoritatile fiscale nu colecteaza si CAS)valabil la data deschiderii ofertelor;Cazier judiciar pentru administratorul/administratorii asa cum sunt mentionati in Certificatul Constatator care semneaza"declaratia privind eligibilitatea"si"declaratia privind calitatea de participant la procedura".În cazul în care declaratiile mentionate sunt semnate de o persoana împuternicita se va prezenta cazierul judiciar pentru aceasta persoana precum si cel al administratorului(ilor) care a împuternicit aceasta persoana;Cazier judiciar al companiei(pt. statele unde e aplicabil).

1. Proiecte de furnizare echipamente
Documente suport (copii dupa contracte/ recomandari/ procesele verbale de receptie preliminara sau finala).

1. Ofertantul (operator economic sau membrii asocierii de operatori economici împreuna) trebuie sa-si demonstreze experienta prin intermediul a maxim 3 proiecte finalizate cu succes în ultimii 3 ani (calculati de la data deschiderii ofertelor), în valoare cumulata de minim 300.000 euro, (valoare ce reprezinta participarea ofertantului în proiectele respective), implementate la nivel regional sau national, în urmatoarele domenii: a) livrare, instalare, punere în functiune, instruirea personalului privind sisteme de comunicatii de date-voce-video si b) livrare, instalare, punere în functiune de echipamente si configurare de servere si sisteme de stocare pentru baze de date ORACLE si/sau SQL 2. Ofertantul (operator economic sau membrii asocierii de operatori economici împreuna) va demonstra accesul la minim 10 specialisti (personal tehnic de specialitate) in domeniul tehnic al echipamentelor IT si de comunicatie 3. Ofertantul trebuie sa aiba implementat un sistem de management al calitatii respectiv ISO 9001 sau echivalent, in domeniul proiectului. In cazul unei asocieri, aceasta conditie trebuie indeplinita de toti membri asocierii
