1) Sealed Tenders are invited by the Chairman, Procurement Committee, District Director of Health Services Office, Nuwaraeliya for the supply of following Medical Equipment.
2) Sealed tenders should be made on a complete set of bidding documents separately for each tender in duplicate.
3) Bidding document can be obtained from the Accounts Branch, District Director of Health Services Office, Nuwareliya on the payment of non-refundable tender as stated above.
4) Tenders should be made on the forms provided by the District Director of Health Services office. Nuwaraeliya only. Tenders in any other forms will be rejected.
5) A certified photocopy of certificate of business registration should be furnished to obtain bidding documents.
6) Tender documents can be obtained from Accounts Branch, District Director of Health Services Office, Nuwaraeliya up to 15th July 2008 between 9.00 a.m. and 3.00 p.m. on normal working days.
7) Tender will be closed and opened at 11.00 a.m. on 16th July 2008 at the office of District Director of Health Services Nuwaraeliya.
