Title

Vulnerability Assessment of the Potable Water Supply Well and Comprehensive Study of the On-site Wastewater Treatment System at the Health Canada Percy E. Moore Memorial Hospital Complex, Hodgson, Manitoba.

Introduction

Health Canada requires the services and technical expertise of a private sector company to complete a comprehensive study of the potable water supply and the on-site wastewater treatment system at Percy E. Moore Memorial Hospital Complex (PMH), Hodgson, Manitoba.
    
Estimated Value

The total value of any contract resulting from this RFP shall not exceed $120,000.00, including travel and living expenses (if applicable), other expenses and all applicable taxes.

Objectives of the Requirement
    
The purpose of this document is to solicit proposals and to establish the Terms of Reference for completing a vulnerability assessment of the potable water supply well and a comprehensive study of the on-site wastewater treatment system at PMH, Hodgson, Manitoba.

Mandatory Requirements

M1.    The Project Manager must demonstrate experience in managing at least three projects related to groundwater well vulnerability, well head protection and source water quality/quantity assessments in the past 5 years by providing a summary of the project manager’s roles and responsibilities for each of the projects, and a summary of scope, objectives and outcomes for each of the projects.
            
M2.    The Project Manager must demonstrate experience in the management and design of at least two projects in the past 5 years in which comprehensive and innovative recommendations were developed for small on-site wastewater treatment systems by providing a summary of the project manager’s roles and responsibilities for each of the projects, and a summary of the scope, objectives and outcomes for each of the projects.
            
M3.    Key Personnel must demonstrate experience in managing two Phase II Environmental Site Assessment projects in the past 5 years in which soil, groundwater, surface water and sediment sampling was completed by providing a summary of the scope, objectives and outcomes for each of the projects.            
M4.    Key Personnel must demonstrate experience in managing two projects in the past 5 years in which geotechnical investigations of the berm and base materials of storage lagoons were undertaken by providing a summary of the scope, objectives and outcomes for each of the projects.            
M5.    Key Personnel must demonstrate experience in the management and/or design of two projects in the past 5 years in which natural wastewater treatment process units were rehabilitated and/or retrofitted to existing wastewater treatment systems by providing a summary of the scope, objectives and outcomes for each of the projects.
            
M6.    Key Personnel must demonstrate experience in the managing two projects in the past 5 years in which condition assessments of process piping and underground infrastructure, including buried process tanks, gravity sewers and forcemains was undertaken by providing a summary of the scope, objectives and outcomes for each of the projects.
            
M7.    The bidder must prove that at least one team member, who will be working on-site in support of the groundwater well vulnerability assessment, has demonstrated experience of at least 3 years in the past 5 years in performing hydrogeological investigations relating to groundwater supply. The individual(s) meeting this mandatory requirement must be clearly identified, along with a summary of their experience. The summary of experience must include a list of representative projects that the individual(s) has/have completed, and a brief description of the capacity in which they were involved with each project.            
M8.    The bidder must prove that at least one team member, who will be working on-site in support of the groundwater well vulnerability assessment, has demonstrated experience of at least 3 years in the past 5 years in performing geotechnical investigations pertaining to the design and/or assessment of facultative lagoons, storage reservoirs and/or retention ponds. The individual(s) meeting this mandatory requirement must be clearly identified, along with a summary of their experience. The summary of experience must include a list of representative projects that the individual(s) has/have completed, and a brief description of the capacity in which they were involved with each project.            
M9.    The bidder must prove that at least one team member, who will be working on-site in support of the wastewater treatment system assessment, has demonstrated experience of at least 3 years in the past 5 years performing wastewater characterization studies, including effluent sampling. The individual(s) meeting this mandatory requirement must be clearly identified, along with a summary of their experience. The summary of experience must include a list of representative projects that the individual(s) has/have completed, and a brief description of the capacity in which they were involved with each project.            
M10.     The Project Manager and Key Personnel must prove that they are certified engineers and/or geoscientists, licensed by their professional organization(s) and entitled to practice in Canada.    
        
M11.    The bidder must provide proof in the form of a valid certificate(s) of accreditation that its proposed laboratory(s) is/are accredited for the analyses of ground and surface water quality, soil and sediment quality and wastewater quality (CAEAL; AIHA-IHLAP; or acceptable equivalents).    
        
M12.    The bidder must provide proof in the form of a valid certificate of insurance that it has Errors & Omissions Liability Insurance coverage.
