Information and formalities for evaluating if requirements are met:
1.Declaratie privind eligibilitatea completata în conformitate cu Formularul A din Sectiunea Formulare - în original;
2.Declaratie privind neincadrarea in situatiile prevazute la art.181 completata in conformitate cu formularul B din Sectiunea Formulare - in original;
3.Certificat de atestare fiscala privind achitarea obligatiilor exigibile catre bugetul de stat si catre bugetele asigurarilor sociale de stat (sanatate, pensii, somaj), eliberat de Directia Generala a Finantelor Publice si Controlul Finantelor de Stat, valabil la data deschiderii ofertei - în original sau copie legalizata.
4.Certificat privind plata taxelor si impozitelor locale, eliberat de Primaria locala, valabil la data deschiderii ofertei - în original sau copie legalizata.

a) bilantul contabil pe 2007, depus la Finantele Publice
Ofertantul trebuie sa prezinte bilantul contabil pe 2007, vizat si înregistrat - în copie
b) ultima balanta de verificare
c) declaratie privind cifra medie de afaceri
Se va completa si depune, în original, Formularul B2 din Sectiunea Formulare.
III.2.3)  Technical capacity
Information and formalities for evaluating if requirements are met:
a)lista a principalelor livrari de produse similare efectuate în ultimii 3 ani, continând valori, perioade de livrare, beneficiari, indiferent daca acestia din urma sunt autoritati contractante sau clienti privati. Livrarile de produse efectuate se confirma prin prezentarea de recomandari din partea autoritatii ori a clientului privat beneficiar.
Se va completa si depune, în original, Formularul C2 din Sectiunea Formulare.
b)certificate sau alte documente emise de organisme abilitate în acest sens, care sa ateste conformitatea produselor;
Produsul trebuie sa îndeplineasca conditiile prevazute de STAS 2529/1988
c)mostre, descrieri si/sau fotografii
Ofertantii trebuie sa prezinte mostra.
d)autorizatie de livrare, în original sau copie legalizata (numai pentru comercianti)
Autorizatia de livrare trebuie sa cuprinda toate informatiile prevazute de Formularul B3 din Sectiunea Formulare, completata de producator.
În situatia în care autorizatia provine de la o alta persoana juridica decât producatorul, trebuie prezentat întreg lantul de autorizari astfel încât sa fie demonstrata legatura dintre producator si ofertant (conform Formular B3.a din Sectiunea Formulare).
e) Aviz valabil emis de Ministerul Muncii si Protectiei Sociale prin Directia Autorizare Agenti Economici Certificare Echipamente Tehnice de Protectie si Grupe de Munca sau declaratie de conformitate -Certificat de examinare EC emis de Ministerul Muncii Solidaritatii Sociale si Familiei - Insitutul National de Cercetare Dezvoltare pentru Protectia Muncii, conform Hotararii 115/26.02.2004, privind stabilirea cerintelor esentiale de securitate ale echipamentelor individuale de protectie si a conditiilor privind introducerea lor pe piata.
f) Standarde de asigurare a calitatii
Se vor prezenta, în copie certificate care atesta implementarea sistemului de calitate ISO 9001, la ofertant sau/si producator, sau dovada ca este în curs de implementare a sistemului de calitate ISO 9001
