Applications on prescribed forms are invited for their Pre-qualification for the supply of following categories items for the year 2008-09.
Pre-qualification form can be obtained from Cashier of this Center from 15-06-2008 to 30-06-2008 during office hours on any working day against cash payment of Rs.500/- (Rupees five hundred only). The form duly filled in and required documents should submit to the office of the Deputy Director (Medical) before 05-07-2008. No application will be entertained after this date.
