Copie dupa statutul sau actul constitutiv.

Ofertantul trebuie sa prezinte o lista cu lucrarile principale realizate în ultimii 5 ani - conform Modelului din sectiunea Formulare - Formular nr. 12F si anexa acestuia.
Se solicita completarea a minim 2 formulare de Experienta Similara (Anexa Formular 12F) pentru lucrari similare si de aceiasi complexitate si minim 2 recomandari de la beneficiari de lucrari similare (Formularul 20)

Declaratie privind echipamentele tehnice, utilaje, instalatii(Formular nr.12H si anexe)
