Title

Concurrent Enhanced Phase I Environmental Site Assessments (ESA) and Environmental Compliance Audits (ECA) at Health Centres in the Ontario and Atlantic Regions.

Introduction

As part of a larger Phase 1 ESA & ECA campaign, Health Canada (HC) requires the services and technical expertise of a private sector company or companies to complete these audits and assessments at 27 health centres in the Ontario and Atlantic Regions.
    
Estimated Value

The total value of any contract resulting from this RFP shall not exceed $399,000.00, including travel and living expenses (if applicable), other expenses and all applicable taxes.

Objectives of the Requirement
    
The purpose of this document is to solicit proposals and to establish the Terms of Reference for conducting the audits and assessments for the facilities listed below, as part of the next series of audits and assessments by environmental consultants across Canada.

The subject facilities in the Ontario Region are:

    •    Akwesasne Health Centre
    •    Tyendinaga Health Centre
    •    Kettle & Stoney Point Health Centre
    •    Oneida Health Centre
    •    Chippewas of the Thames Health Centre
    •    Chippewas of Sarnia Health Centre
    •    Walpole Island Health Centre
    •    Gane Yohs Health Centre
    
    The subject facilities in the Atlantic Region are:

    •    Annapolis Valley Health Centre
    •    Acadia (Gold River) Health Centre
    •    Bear River First Nation Health Centre
    •    Lennox Island Health Centre
    •    Madawaska Maliseet Health Building
    •    Pictou Landing Health Centre
    •    Abegweit First Nation Mi’kmaq Wellness Centre
    •    Chapel Island Medical Centre (including dental clinic)
    •    Membertou Wellness Centre
    •    Acadia (Yarmouth) Health Centre
    •    Shubenacadie Health Centre
    •    Glooscap Health Centre
    •    Eskasoni Health Centre
    •    Waycobah Health Centre (including dental clinic)
    •    Conne River Health Centre
    •    Eel River Bar Health Centre
    •    Mani Ashini Health Centre
    •    Millbrook Health Centre
    •    Wagamatcook Health Centre (including dental clinic)

Mandatory Requirements

Attention Bidders: Write beside each of the criteria the relevant page number(s) from your proposal which addresses the requirement identified in the criteria.

M1.    The bidder must provide a table that outlines the personnel to be assigned to the project including names, their individual roles and responsibilities within the project, qualifications and experience to undertake the required skills. The individual(s) meeting this mandatory requirement must be clearly identified, along with a summary of their experience. The summary of experience must include a list of representative projects that the individual(s) has/have completed, and a brief description of the capacity in which they were involved with each project.
            
M2.    The bidder must prove that at least one team member (per team if there will be multiple teams conducting on-site work simultaneously), who will be working on-site during the audits and assessments, has demonstrated experience of at least 3 years, within the past 5 years, in Environmental Compliance Auditing. The individual(s) meeting this mandatory requirement must be clearly identified, along with a summary of their experience. The summary of experience must include a list of representative projects that the individual(s) has/have completed, and a brief description of the capacity in which they were involved with each project.
            
M3.    The bidder must prove that at least one team member (per team if there will be multiple teams conducting on-site work simultaneously), who will be working on-site during the audits and assessments, has demonstrated experience of at least 3 years, within the past 5 years, in Phase One Environmental Site Assessment. The individual(s) meeting this mandatory requirement must be clearly identified, along with a summary of their experience. The summary of experience must include a list of representative projects that the individual(s) has/have completed, and a brief description of the capacity in which they were involved with each project.    
        
M4.    The bidder must prove that at least one team member (per team if there will be multiple teams conducting on-site work simultaneously), who will be working on-site during the audits and assessments has demonstrated and recent (within the past 2 years) experience in indoor air quality issues, including microbial contamination investigation/survey. The individual(s) meeting this mandatory requirement must be clearly identified, along with a summary of their experience. The summary of experience must include a list of representative projects that the individual(s) has/have completed, and a brief description of the capacity in which they were involved with each project.
            
M5.    The bidder must prove that at least one team member (per team if there will be multiple teams conducting on-site work simultaneously), who will be working on-site during the audits and assessments has demonstrated experience (within the past 2 years) in asbestos identification and sampling. The individual(s) meeting this mandatory requirement must be clearly identified, along with a summary of their experience. The summary of experience must include a list of representative projects that the individual(s) has/have completed, and a brief description of the capacity in which they were involved with each project.            
M6.    The bidder must prove that at least one team member (per team if there will be multiple teams conducting on-site work simultaneously), who will be working on-site during the audits and assessments has demonstrated experience (within the past 2 years) in potable water sampling. The individual(s) meeting this mandatory requirement must be clearly identified, along with a summary of their experience. The summary of experience must include a list of representative projects that the individual(s) has/have completed, and a brief description of the capacity in which they were involved with each project.        
    
M7.    The bidder must provide the name of its proposed laboratory(s) with accreditation of analyses for asbestos and potable water quality (CAEAL; AIHA-IHLAP; or acceptable equivalents) and proof by way of a certificate that they are accredited.    
        
M8.    The bidder must provide a copy of their valid Errors & Omissions Liability Insurance coverage.
