II.1) DESCRIPTION

Provision of a Hospital Discharge Screening Service (at The Royal
Bournemouth and Christchurch Hospitals).

II.1.5) Short description of the contract or purchase(s): The Bournemouth
Borough Council on behalf of Dorset County Council, Hampshire County
Council, Borough of Poole and the Royal Bournemouth & Christchurch
Hospitals NHS Foundation Trust, wish to invite suitably experienced
Organisations / Registered Charities to tender for the Provision of a
Hospital Discharge Screening Service. The service involves working with
patients and carers who will be referred to the Provider by clinical
staff, providing advice and guidance on services available in the
community, setting up certain services for patients and referring some
patients onto social work services as necessary. The service involves
working with patients, carers, social workers and clinical staff in
identifying Patients/Service Users ready for discharge from Hospital,
providing advice and guidance on services available in the community. It
is envisaged that the Contract will commence 1.4.2009 for 3 years, with an
option for the Council to extend for a further period of up to 2 years, in
one-yearly increments. TUPE Regulations may apply.

II.2.1) Total quantity or scope: Contract is being tendered on behalf of
Bournemouth Borough Council, Borough of Poole, Hampshire County Council,
Dorset County Council and Royal Bournemouth & Christchurch Hospitals NHS
Foundation Trust who are currently joint service commissioners.
Estimated value excluding VAT:

Description of these options: There is an option to extend the contract by
up to 2 years in yearly increments (subject to provider performance).
Provisional timetable for recourse to these options: in months: 36 (from
the award of the contract)

II.3) DURATION OF THE CONTRACT OR TIME-LIMIT FOR COMPLETION: Duration in
months: 60 (from the award of the contract).
SECTION III: LEGAL, ECONOMIC, FINANCIAL AND TECHNICAL INFORMATION
III.1) CONDITIONS RELATING TO THE CONTRACT
III.1.2) Main financing conditions and payment arrangements and/or
reference to the relevant provisions regulating them: Payment for the
service will be made 28 days in arrears of a monthly undisputed Invoice.
The current Provision of a Hospital Discharge Screening Service, under a
Joint-Purchasing Agreement is funded as below (for the period 1.4.2008 to
31.3.2009): Bournemouth Borough Council 45 %; Dorset County Council 23 %;
The Royal Bournemouth & Christchurch Hospitals NHS Foundation Trust 19 %;
Hampshire County Council 8 % and Borough of Poole 5 %; Each Authority is
fully responsible and separately liable for payment of the service they
receive. For the avoidance of doubt no Authority will be liable for
another Authority’s debt.
