II.1.4) Short description of the contract or purchase(s): The main aim of
this service is to have in place one resource to meet all needs,
predominantly for people in South Ribble, be it only advice or support, to
the clinical assessment, rehabilitation or day respite for carers.
Lancashire County Council Adult and Community services will offer an
enhanced day care service, 5 days per weeks Monday to Friday initially,
and will support the development of a carers cafe. The enhanced day
service will provide an assessment function, as well as a rehabilitation
programme. A stimulating surrounding and a strong educational role for
both the individual and their carer. The centre will be able to assist a
small number of people who require carers respite. Lancashire Care trust
will move their present hospital based services such as the day hospital
assessment and memory clinics, into the new venue. The community
psychiatric nurses and occupational therapists will also work from the
centre together with social workers from the area. The Voluntary Sector
will base their flexible outreach service and in partnership with the
Alzheimer soc, support the reception area, carers cafe and carers support
and advice section. Social work services.
