Declaratie privind lista pricipalelor lucrari similare cu cerintele caietului de sarcini executate in ultimii 5 ani, care sa contina informatii despre cel mult 3 contracte de lucrari similare care din punct de vedere fizic si/sau valoric sa insumeze cel putin 50% din volumul fizic sau valoric al viitorului contract;Declaratie privind efectivul mediu anual al personalului angajat si a cadrelor de conducere;Declaratie referitoare la utilajele, instalatiile, echipamentele tehnice de care poate dispune operatorul economic pentru indeplinirea corespunzatoare a contractului de lucrari ce urmeaza a fi atribuit;Informatii referitoare la studiile, pregatirea profesionala si calificarea personalului de conducere, precum si a persoanelor responsabile pentru executia contractului;Ofertantii vor face dovada incadrarii cu deserventi calificati, specializati si autorizati pentru utilajul/utilajele mentionat/mentionate in Anexa nr.1 a caietului de sarcini;Dovada detinerii autorizatiilor ISCIR pentru automacarale, inscrise in termenul scadent.
