Declaratie privind eligibilitatea -conform modelului anexat in sectiunea III-Formular 12 A
Declaratia privind neîncadrarea în prevederile art. 181 din OUG 34/2006 - conform modelului anexat. in sectiunea III- Formular 12 B
Certificate constatatoare privind îndeplinirea obligatiilor exigibile de plata a impozitelor si taxelor catre bugetul de stat si cele locale (formulare-tip eliberate de autoritatile competente din tara în care candidatul/ofertantul este rezident) în conformitate cu Codul fiscal- original sau copie legalizata

Se solicita autorizatia de functionare, precum si orice alt document edificator considerat necesar pentru dovedirea apartenentei la categoria profesionala impusa de indeplinirea contractului ;

Documente edificatoare care sa dovedeasca o forma de înregistrare ca persoana juridica sau de înregistrare/atestare ori apartenenta din punct de vedere profesional, în conformitate cu prevederile legale din tara în care candidatul/ofertantul este rezident. Documentele trebuie sa dovedeasca faptul ca ofertantul are prevazut în obiectul de activitate servicii de tipul celor supuse ofertarii.
Documentele vor fi prezentate în traducere legalizata.
III.2.2)  Economic and financial capacity
Information and formalities for evaluating if requirements are met:
Informatii generale -conform modelului anexat in sectiunea III- Formular 1
Bilantul contabil din anul precedent (2007), vizat si înregistrat de organele competente-copie.
Pentru persoanele juridice straine, se solicita ultimul bilant contabil, iregistrat la organele competente in tara respectiva.
Nivelul cifrei de afaceri medie solicitata ca cerinta minima, pe ultimi 3 ani:
Valoarea minima impusa este de: 1.600.000EURO, respectiv 5.964.160 LEI
Cifra medie anuala se va calcula prin impartirea valorii cifrei de afaceri exprimata in lei la cursul euro mediu anual practicat de BNR pentru anul respectiv. Se vor lua in calcul urmatoarele cursuri:
2005: 1 Euro = 3,6234 lei;2006: 1 Euro = 3,5245 lei;2007: 1 Euro = 3,3373 lei
III.2.3)  Technical capacity
Information and formalities for evaluating if requirements are met:
Declaratie privind lista principalelor lucrari executate in ultimii 5 ani- conform modelului anexat in sectiunea III-Formular 12 F
Obligativitatea prezentarii dovezii indeplinirii si finalizarii, in ultimii 5 ani, a cel putin unui contract (Formular Experienta similara -conform modelului anexat in sectiunea III- Formular 2) care a avut ca obiect executarea unor lucrari similare si care a avut valoarea egala sau mai mare decat valoarea minima impusa de:
-de 160.000 EURO, respectiv 596.416 lei
Pentru acesta se va prezenta recomandare- conform modelului- Recomandare - anexat in sectiunea III-Formular 3A;
Informatii privind -Personal ce va fi angajat in proiect- trebuie completat de candidat/ ofertant -conform modelului anexat in sectiunea III- Formular 4.
-Curriculum Vitae personalului ce trebuie angajat in contract, -conform modelului anexat in sectiunea III-Formular 5, trebuie completat de catre ofertant/ candidat
Dosar de prezentare a sistemului de conducere si de asigurare a calitatii lucrarilor

Lista cuprinzând subcontractantii, însotita si de acordurile de subcontractare; subcontractantii care urmeaza sa îndeplineasca mai mult de 10% (în exprimare valorica) din contractul de achizitie publica trebuie sa prezinte obligatoriu cu propriile date documentele solicitate la punctul V.1), V.2) V.3) si V.4) din fisa de date a achizitiei .
DECLARATIE privind partea/partile din contract care sunt îndeplinite de subcontractanti si specializarea acestora conform modelului anexat in sectiunea III-Formular nr.12 G.
