SITE DEVELOPMENT AND CONSTRUCTION OF BAIS CITY SLAUGHTERHOUSE PROJECT

This Bid Bulletin No. 02 is issued to amend certain items and provisions in the Bidding Documents of the abovementioned project. This shall form an integral part of the Bidding Documents.

Notice is hereby given to the bidders of the Bais City Slaughterhouse Project that such project falls under the mixed procurement category; and that in case of joint venture, bidders shall be required to be duly registered with the Construction Industry Authority of the Philippines (CIAP) for a Special Consortium Permit; provided that the equipment fabricator-installer is duly accredited by the National Meat Inspection Service (NMIS) and the contracting parties who formed a joint venture shall both be subjected to eligibility check (for civil works - equipment fabricator/installer, as the case may be), pursuant to the guidelines setforth in R.A. 9184.

The inclusion of the name of the equipment fabricator-installer in the organizational chart of the bidder as the bidder's employee is explicitly not allowed in lieu of a joint venture agreement.

The equipment fabricator/installer of a joint venture bid is also required to submit the following eligibility documents, viz:

1. DTI Business Name Registration in the case of Single Proprietorship; or SEC Registration Certificate, in the case of Partnership or Corporation;
2. Valid and Current Mayor's Permit/Municipal License to operate a business;
3. BIR Registration Certificate, which contains the Taxpayer's Identification Number;
4. Sworn Statement of the prospective bidder that it is not "blacklisted" or barred from bidding by the government or any of its agencies, offices, corporations or LGUs, and that it is not included in the Consolidated Blacklisting Report, once this is issued by the GPPB;
5. Tax Clearance Certificate issued by the BIR Main Office and Income or Business Tax Returns filed through the EFPS (E.O. 398, s. 2005), if applicable;
6. Joint Venture Agreement, if applicable; shall contain a statement on who the joint venture/association has constituted and appointed as the lawful attorney-in-fact to sign the contract, if awarded the project, and on who among the members is the lead representative of the joint venture.
7. NMIS Accreditation, Single -A Category for Region 7 and Double-A Category for outside Region 7;
8. Letter authorizing the BAC or its duly authorized representatives to verify any or all of the documents submitted for the eligibility check;
9. Sworn statement that each of the documents submitted in satisfaction of the eligibility requirements is an authentic and original copy, or a true and faithful reproduction or copy of the original, complete, and that all statements and information provided therein are true and correct.

For guidance and information of all concerned, the equipment and materials specifications in the bidding documents shall prevail over the specifications printed in the plans, in case of conflict or difference.

The equipment specifications in the bidding documents are minimum requirements; and the bidder is allowed to introduce design variations provided that such variations are upgraded designs and meet NMIS standards.
