Information and formalities for evaluating if requirements are met:
Cazier judiciar sau declaratie privind eligibilitatea (formular 12A, Ord ANRMAP 155/2006);Certificat emis de Registrul Comertului si formular 12B din Ord. 155 al ANRMAP (neincadrarea in art.181/OUG 34/2006, modif de OU 94/2007); confirmarea platii taxelor locale si a impozitelor la bugetul consolidat, valabile la data deschiderii ofertelor (copii); persoane fizice/ juridice romane/straine-copie Certificat emis de Oficiul Registrul Comertului de pe langa tribunalele teritoriale (inregistrarea- fata verso, obiectul de activitate).

Information and formalities for evaluating if requirements are met:
Bilantul contabil, la 31 decembrie 2007, vizat si inregistrat de organele competente; cifra medie de afaceri pe ultimii 3 ani - valoare minima impusa 980.000 lei ( ½ pentru IMM-uri).Pentru operatorii economici care au o durata de functionare mai mica de 3 ani, cifra medie de afaceri se va calcula proportional cu durata de functionare.

Informatii: obligatii contractuale in desfasurare; personal tehnic de specialitate pentru asigurarea controlului calitatii; pregatirea profesionala si calificarea personalului de conducere precum si ale persoanelor responsabile pentru indeplinirea contractului de lucrari (sefi de lucrare proprii, autorizati minim gr.III; RTE atestat MLPTL / MTTC; personal autorizat si dotat corespunzator factorilor de risc cumulati pentru lucrare); dotarea tehnica (laborator autorizat gradul II sau contract incheiat cu un laborator autorizat gradul II pentru prelevare probe si efectuare incercari la materialele puse in opera); partile si proportia in care contractul de lucrari urmeaza sa fie indeplinit de catre subcontractanti; experienta similara (min un contract de lucrari similare exec. si finalizat in ultimii 5 ani si recomandarea aferenta); asumarea raspunderii de a monta echipamentele pentru sistemul de efractie.
Certificatul sist de managementul calitatii, documente emise de organisme acreditate, care confirma certificarea sistemului calitatii; descrierea sistemului de management al calitatii cu precizarea particularitatilor de aplicare la lucrarile care fac obiectul prezentului contract.
