-Declaratie ca ofertantul nu este asociat si nu a fost asociat in trecut, direct sau indirct cu consultantul sau orice alta entitate care a elaborat proiectul tehnic, specificatiile tehnice sau alte documente pentru proiect sau care a fost propus manager de proiect pentru contract;
-Declaratie ca ofertantul nu se afla su incidenta unei declaratii de neeligibilitate pentru practici corupte si frauduloase emise de banca in conformitate cu sub-clauza 35;
-Copii legalizate ale documentelor originale ce definesc constituirea sau statutul legal, locul de inregistrare si locul principal de desfasurare a activitatii
