OBJECTIVE
USDA, Rural Development requires the services of a company that is EPA Certified to perform lead hazard control services in New York. The property is an Agency inventory property and all identified lead hazards must be abated in accordance with the Residential Lead-Based Paint Hazard Reduction Act of 1992, commonly known as Title X HUD Rule 1012 and 1013 of Title X, activated in September 2000. The specific reference is Subpart C -Disposition of Residential Property Owned by a Federal Agency Other than HUD, which states the Federal agency shall abate all identified lead-based paint hazards in accordance with 40 CFR 745.227. Abatement is completed when clearance is achieved in accordance with 40 CFR 745.227.
SERVICES AND PRICES
PRICE SCHEDULE
The Contractor shall provide the following services at the price listed for each job. The quoted prices shall include all wages, indirect costs, travel (mileage), overhead, general and administrative expense, profit, materials, labor, tools, supplies, permits, etc. All items removed from property are to be disposed of in accordance with appropriate regulations and lead hazard based paint requirements. See attached floor layouts (not drawn to scale).
LOCATION: 2667 Co Rte 57, Fulton, NY 13069
All lead based paint to be removed as listed below. On exterior areas, ground cover will be used to prevent ground contamination. No soil to be removed unless contamination occurs. Signs will be placed indicating that lead based paint removal is being done. Perform specialized cleaning using HEPA vacuum and wet cleaning methods throughout the house.
CONTRACTOR IS TO COMPLETE
1. Foyer 1 Wall 1--Remove interior door unit (door to basement) and replace with 32 x 74.5 new hollow core wooden interior door. Replace interior lockset with Kwikset privacy lockset, polished brass or comparable interior privacy lock. Door is to be left unpainted/stained. Door hinges must also be removed/replaced with hinges matching privacy lockset in color.

2. Foyer 1 Wall 2 --Remove exterior door unit and install plywood approx 32 x 74.5over opening on the exterior side and cover with siding. Install plywood 32 x 74.5 over the opening on the inside. Do not cover or paint. In between plywood install 4 Owens/Corning Fiberglass rolled insulation.

3. Foyer 1 Wall 4Remove existing door jamb components and install new pine door jamb components 32 x 3.5 x 80 Components are to be left unpainted/unstained.

4. Foyer 1 Wall 1Remove existing door jamb components and install new pine door jamb components 32 x 3.5 x 80. Components are to be left unpainted/stained.

5. Foyer 1 Wall 4Remove existing door and install new exterior door unit. Replace with steel insulated door with no window. Door measures approx. 30 x 80. Replace exterior lockset with Kwikset Juno combo pack, polished brass or comparable exterior lock set. Door hinges are also to be removed/replaced with hinges matching the lockset in color.

6. Foyer 1 Wall 4 and Wall 2 (door molding)--Remove existing door moldings and replace only door moldings on wall 4 with new door molding. Use 1 x 4 pine molding. Door moldings on Wall 2 are to be removed but not replaced. New molding is to be left unpainted/stained.

7. Foyer 1 Wall 4 -Stair TreadCompletely remove all 4 stair treads and replace with 4 new stair treads. Install medium grade pine approx. 10.5 x 40. New treads are to be left unpainted/stained.

8. Foyer 1 Wall 4 Stair StringerRemove existing stair stringers and install new stair stringers. Install medium grade pine approx. 39 x 10. New stringers are to be left unpainted/stained.

9. Foyer 1 FloorRemove existing flooring and install new flooring. Install medium grade pine approx. 38 x 34.5. New flooring is to be left unpainted/stained/varnished.

10. Foyer 1 Wall 1, 2, 3, and 4Remove existing window units (to include casings, jambs, sills, sash, wells, moldings) and install with new vinyl clad window units (to include installation of new casings, jambs, sills, sashes wells and interior/exterior moldings ) with white double hung, double glazed, argon filled windows with at least an R-4 rating, low-E (Pella or equal) with screen for the following:

11. Foyer 2 Wall 4 Stair TreadRemove all 5 stair treads and install 5 new stair treads. Install medium grade pine approx. 10.5 x 40. New treads to be left unpainted/stained/varnished.

12. Foyer 2 Wall 4 Stair RiserRemove all 5 stair risers and install 5 new stair risers. Install medium grade pine approx 7x 40. New risers are to be left unpainted/stained/varnished.

13. Foyer 2 Wall 4 FloorRemove existing flooring and install new flooring. Install medium grade pine approx. 35.5 x 37. New flooring is to be left unpainted/stained/varnished.

14. Foyer 2 Wall 1 Threshold--Remove existing threshold and replace with 2 thick x 8 wide treated lumber approx 32 long. New threshold is to be left unpainted/stained/varnished.

15. Foyer 2 Wall 4 Newel PostRemove old newel posts (3 posts in all) and install new newel posts measuring 4x 4 x 52 4x 4 x 53 and 4x 4 x 44. The new newel posts are to be left unpainted/stained/varnished.

16. Foyer 2 Wall 4 Baluster and HandrailRemove existing baluster and replace with new pine baluster approx. 2 x 2 x 36 and 2 x 2 x 16 and 2 x 2 x 10. Remove and replace existing handrail using 1 x 4 pine 34 16 and 10. New handrails/baluster is to be left unpainted/stained/varnished.

17. Porch Wall 3 Remove existing door and install new exterior door unit. Replace with steel insulated door with no window. Size of door approx. 34 x 80. Replace exterior lockset with Kwikset Juno combo pack, polished brass or comparable exterior lock set. Door hinges are also to be removed/replaced with new hinges matching the lockset in color.

18. Porch Wall 3 Door Molding--Remove existing interior door molding and install new 1 x 4 pine molding. New interior molding is to be left unpainted/stained/varnished. Exterior molding is being replaced with vinyl j-channel to match siding in color.

19. Porch Walls Remove existing clapboard siding and door molding and install white double 5 clapboard style vinyl siding with vinyl j channel, etc. on wall #2 approx size 9H x 6w and wall #3 approx size 9H x 5w.

20. Porch Wall 1 Stair Handrail and ColumnRemove existing column and hand railing/baluster unit. Install new pine column measuring 99 x 6 square and hand railings measuring 64 long x 32 high and 3-4 wide. Paint with 2 coats premium white exterior latex paint (Glidden, Valspar or equal).

21. Porch Wall 4 Ceiling Remove existing clapboard and replace with 5 x 6 vinyl wainscoting (CertainTeed or equal) and seal all seams/edges.

22. Porch Wall 3--Remove existing threshold and install new pine threshold 6 x 36. New threshold is to be left unpainted/stained/varnished.

23. Main Basement Wall 1 (southeast corner)--Remove existing paneling. Do not replace. Wall 2 (south east corner)--Remove existing paneling. Do not replace. Basement #2 Wall 2 (south wall behind stairs)--Remove existing wall coverings. Do not replace.

24. Basement Wall 3 Stair TreadRemove old stair treads (3 treads) and install new stair treads. Install medium grade pine approx. 10 x 40. New treads to be left unpainted/stained/varnished.

25. Basement Wall 3 Stair Risers Remove old stair risers (3 risers) and install new stair risers. Install medium grade pine approx. 7 x 8. New risers are to be left unpainted/stained/varnished.

26. Basement Wall 3 Stair StringerRemove old stair stringers (2 stringers) and install new stair stringer. Install medium grade pine approx. 10x 10. New stringers are to be left unpainted/stained/varnished.

27. Basement #2 Wall 3 WindowsRemove existing window units (2 windows) measuring approx. 28 x 25and install new vinyl clad white basement windows units. Remove and replace existing interior molding with new 1 x 4 pine moldings. Interior replacement molding is to be left unpainted/stained/varnished.

28. Basement Hand RailingRemove existing hand railing and replace with 511 long and approx. 3 diameter rounded pine hand railing. New handrail is to be left unpainted/stained/varnished.

29. Garage All 4 Exterior WallsRemove existing clapboard siding, all corner boards, soffits, overhead door frame and install white double 5 vinyl siding with trim on all 4 exterior walls, measuring approximately 20.3 x 124. Vinyl j channel, drip edge and soffits, etc. Replace facia with new treated 1 x 5 lumber and trim with white aluminum (CertainTeed or equal) and seal all seams/edges.

30. Garage Wall 2, Wall 3, Wall 4--Remove existing window units (to include casings, jambs, sills, sash, wells, moldings) and install with new vinyl clad window white double hung windows units approx. 18 x 23. (Installation includes of new casings, jambs, sills, sashes and wells) Exterior moldings to be replaced with white vinyl j. channel to match siding. Interior moldings to be replaced with 1 x 4 pine molding. Interior molding is to be left unpainted/stained/varnished while exterior molding is to be primed and painted with two coats of white Glidden, Valspar or equal exterior latex paint.

31. House Exterior Wall 4--Remove existing door and install new exterior door unit. Replace with steel insulated door with no window. Size of door approx. 30 x 80. Replace exterior lockset with Kwikset Juno combo pack, polished brass or comparable lock set. Door hinges are also to be removed/replaced with hinges matching the lockset in color.

32. House Exterior Wall 3 and Wall 4 Door Molding--Remove existing molding and replace with white vinyl j channel to match siding.

33. House Exterior Wall 1, 2, 3 and 4 Window MoldingRemove existing molding and replace all window molding with white vinyl j. channel to match siding.

34. House Exterior Wall 4 Ceiling--Remove existing roof, roof supports and roof over back door and replace with 32 aluminum awning.

35. House Exterior Wall 1 ShuttersRemove existing shutters and replace with 2 sets of Green vinyl shutters 55 x 16 to match as closely as possible other existing shutters.

36. House Exterior Wall 2, Wall 3 and Wall 4Remove existing cellar storm windows to include casings, jambs, sills, sash and wells. Install new vinyl cellar window units and components. Use white double glazed, windows. Cellar windows on Walls 2 and 3 are approx. 28 x 25 on Wall 4 is approx 17 x 31

37. House Exterior Wall 4--Remove exterior closet door unit. Install approx. 18 x 18 plywood over opening and cover with siding to match existing siding.

38. House Exterior Walls 1, 2, 3, and 4--Remove existing clapboard siding and door molding and install white double 5 clapboard style vinyl siding with vinyl j channel, drip edge and soffits, etc. on all four walls. Replace facia with new treated 1 x 5 lumber and trim with white aluminum (CertainTeed or equal) and seal all seams/edges.

39. Foyer 3 Wall 1 Door-Remove existing door and install new exterior door unit. Replace with steel insulated door with no window. Size of door approx. 34 x 80. Replace exterior lockset with Kwikset Juno combo pack, polished brass or comparable exterior lock set. Door hinges are also to be removed/replaced with new hinges matching the lockset in color.

40. Foyer 3 Wall 4 Molding-Remove and replace existing molding. Install new 1 x 4 pine molding. New molding is to be left unpainted/stained/varnished. Exterior molding is being replaced with vinyl j-channel to match siding in color.

41. Foyer 3 Wall 2 Shelf Completely remove shelf. No new shelf will be installed.

42. Kitchen Wall 4 Kitchen Cabinets and Cabinet Shelves Remove existing cabinets and shelves. Install new kitchen cabinets and cabinet shelves with low-medium grade cabinets and shelves. The 2 cabinets above the sink measure approx. 45W/ 24D/40H. The standard base cabinets measure 34.5H/118L/24D. The Small Cabinet next to stove measures approximately 12W / 34.5H /24D. The Pantry measures approximately 20W/ 84H24D

43. Kitchen Baseboards--Remove existing baseboards and install new baseboards. - 94 x 7x 2x 2. New baseboards are to be left unpainted/stained/varnished.

44. Bathroom Wall 4 Bathroom Door JambRemove existing door jamb components and install new pine door jamb components. Measure approximately 28 x 80. Newly replaced components are to be left unpainted/stained/varnished.

45. Bathroom Wall 4Remove existing ceiling and install new ceiling. Install green board in bathroom ceiling approx. 410 x 89and paint with two coats of white semi gloss paint (Glidden, Valspar or equal).

46. Bathroom--Remove existing walls 1, 2 and 4 and install sheet rock, mud, tape and prime for painting with two coats of white semi gloss paint (Glidden, Valspar or equal).

47. Stairway Wall 3 Door JambRemove existing door jamb components and install new pine door jamb components 32 x 3.5 x 80. Newly replaced components are to be left unpainted/stained/varnished.

48. Stairway 2 Wall 3 Floor Remove existing floor and install solid hardwood unfinished oak flooring approx. x 2 to match as close as possible to remaining floor. Approx. size of area to be replaced 194 x 104 .Clean using a wet HEPA vacuum method. Finish with clear Varathane Floor Finish Varnish or, comparable product.

49. Bedroom 1 Wall 1 Door JambRemove existing door jamb components and install new pine door jamb components measuring approximately 32 x 3.5 x 80. Remove door moldings and replace with new door molding. Install new 1 x 4 pine molding. Newly replaced components are to be left unpainted/stained/varnished.

50. Bedroom 2 Wall 1 Baseboard-Remove existing baseboards and install new baseboards. Approx. 94 x 123. Closets are 20x 36 and 20 x 40. Remove door moldings and replace with new door molding. Install new 1 x 4 pine molding. Newly replaced door molding is to be left unpainted/stained/varnished.

Workers must follow removal requirements as listed under Abatement 24 CFR 35.120(b). Safe Work Practices 24 CFR 35.1350, Worksite Preparation and Containment 24 CFR 35.1345, Worksite Clean up 24 CFR 35.1350(c) and Clearance Examination 24 CFR 35.1340. In those areas where soil is removed, replacement topsoil, mulch or similar material will be brought in its place. Contaminated soil, debris, containment materials and lead dust will be properly disposed of using a method that is acceptable to EPA and HUD. Work will be completed using lead safe work practices to the extent that clearance is achieved. Final payment will be made once the clearance report states that all lead hazards have been abated.
